What the fill volume in a Wegovy pen actually tells you

The MHRA-approved Wegovy semaglutide injection comes in five dose strengths: 0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg and 2.4 mg, and since April 2026, a dedicated 7.2 mg single-dose pen approved for adults with a BMI of 30 or above. Each pen is pre-filled and delivers one weekly dose. The volume of liquid inside varies by strength because semaglutide is formulated at different concentrations across the dose ladder, and a higher dose does not simply mean a larger fill.

Practically, this means two pens at different steps can look identical in your hand yet contain quite different amounts of active medicine per millilitre. Novo Nordisk publishes the full concentration data in the Summary of Product Characteristics, which you can check on the Electronic Medicines Compendium, it takes under a minute to search by product name and pull up the exact specification for your current dose. Why does the volume matter for cost? Because some advertised prices are quoted per pen rather than per month, and the number of pens in four weeks stays constant at one, but the physical contents, and therefore the manufacturing cost passed through the supply chain, differ. The NICE appraisal of semaglutide for weight management (NICE TA875) established the clinical framework, but most people accessing Wegovy privately sit outside that NHS route entirely. Private pricing reflects the licensed UK supply chain, which is separate from NHS procurement and subject to standard commercial margin at each stage.

What changes a headline price significantly is what surrounds the pen. Some providers quote the medicine cost alone and add a consultation fee, a prescription charge, and a delivery fee at checkout. Others bundle some but not all. A figure that looks attractive for the starter dose can feel very different once you account for the maintenance-dose price at 2.4 mg, where Eli Lilly's September 2025 list-price increase pushed Mounjaro's comparable pen from around £122 to around £330, a move that sent ripples through private-market pricing for GLP-1 medicines broadly, including Wegovy.
