In the first week on Mounjaro, most people lose very little — sometimes nothing at all. The 2.5mg starting dose is a tolerability phase, not a therapeutic one; your body is adjusting, not yet responding to full appetite suppression. Meaningful weight change typically takes four to eight weeks to become visible. That said, understanding what the early weeks are actually for makes the whole process far less frustrating — and that is probably exactly why you are reading this. Mounjaro (tirzepatide) is a prescription-only weight-management medicine, and a clinician, not a bathroom scale, decides whether it is right for you.

What the first-week numbers really mean, and how pricing fits in

Step 1: Understand what week one is actually doing

The licensed Mounjaro schedule opens at 2.5mg. That first pen exists to let your digestive system settle, not to drive weight loss. Nausea, bloating and changes in appetite are common in the early weeks; the lower dose keeps them manageable while your body adapts to tirzepatide's dual GIP and GLP-1 action.

So if you weigh yourself on day seven and the number has barely moved, that is the medicine working as designed. The first week is about tolerance, not transformation. Any fluctuation you see is more likely water balance and food timing than genuine fat loss. Some people do notice their appetite drop noticeably in week one; others feel almost nothing yet. Both are normal.

The clinical trials that underpinned NICE's recommendation of tirzepatide (SURMOUNT-1, published in the New England Journal of Medicine) measured outcomes over 72 weeks, with an average body-weight reduction of around 20–21% at the 15mg maintenance dose. That is a 72-week number across thousands of participants. Week one contributes almost nothing to it. Patience at the start pays off later.

Step 2: Know what realistic early progress looks like, and when to expect it

For most people in week one of Mounjaro, the honest answer is between 0 and 2–3 lbs, if anything. Water weight can cause a temporary dip that reverses by week two. Genuine appetite reduction, which is where the medicine earns its place, tends to build through weeks two to four as the body adjusts.

The first full month on Mounjaro is a better unit to track. By week four, many people have had a real reduction in appetite and are eating noticeably less without actively trying. That is also when the prescriber begins to consider whether to step up to 5mg. A prescriber, not a schedule on a website, makes that call based on how you are tolerating the current dose.

If you want a broader picture of average weight loss across the full Mounjaro course, the clinical evidence is more encouraging, but it takes months, not days, to arrive.

Step 3: Factor pricing into your decision sensibly

Private Mounjaro treatment in the UK is priced per pen, per month. Since Eli Lilly revised its UK list price in September 2025, typical private-market prices have ranged roughly £149 to £375 per month depending on the dose and the provider, widely reported figures that give you a realistic frame, not a guarantee of what you will pay anywhere specific.

Headline prices from some providers cover the pen and little else. Consultation fees, prescription charges, delivery costs and access to clinical aftercare are often separate line items. Understanding what Mounjaro actually costs means adding those up before comparing. A lower number that excludes prescription and clinical review is not necessarily the better deal.

At nume, one price covers the consultation, the prescription, the medicine, next-working-day tracked delivery and seven-day aftercare support. We are not the cheapest option and we are not trying to be. Every repeat order goes through a fresh clinical review before dispatch, no auto-renewals, no subscription you forget you signed up for. Our clinical team, including our prescribers, is there specifically to support you through the titration phase, which is precisely when most people have questions.

Step 4: The right metric for a prescription medicine is not the price tag

For any prescription-only medicine, the lowest price is simply the wrong thing to optimise for. Mounjaro has to be prescribed following a proper clinical assessment; that is UK law under the Human Medicines Regulations, and it is also just good sense for a medicine that interacts with gut hormones and requires careful dose titration.

The MHRA has warned publicly about counterfeit weight-loss pens sold online without clinical oversight, including a 2025 raid on an illegal UK manufacturing site producing fake tirzepatide pens. The guidance from the MHRA Drug Safety Update is consistent: obtain these medicines through a pharmacy you can verify on the GPhC register. You can check any online pharmacy (including ours (GPhC registration 9012878)) directly at the GPhC pharmacy register.

The treatment options available through nume are prescribed only when a clinician judges them clinically appropriate. If you are ready to see whether Mounjaro fits your situation, start your free consultation and a prescriber will review your answers the same day.
