The route to Mounjaro when your BMI is below 30, what the licensed criteria actually say

Does the licensed BMI threshold really allow for under 30?

It does, under specific conditions. The UK licence for Mounjaro covers two groups: adults with a BMI of 30 or above, and adults with a BMI of 27 or above who have at least one weight-related comorbidity. That second group is the relevant one here. Conditions that commonly qualify include type 2 diabetes, prediabetes, hypertension, dyslipidaemia (high cholesterol or triglycerides), obstructive sleep apnoea and osteoarthritis. One detail worth knowing: for people from South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, UK clinical guidance recognises that health risks occur at lower BMI values, and thresholds are typically 2.5 kg/m² lower as a result. So a BMI of 27 in a white European patient and a BMI of 27 in a South Asian patient are not assessed in exactly the same way.

What is the legal route to a Mounjaro prescription in the UK?

Mounjaro is a Prescription-Only Medicine. The only legal route is a prescription issued by a registered prescriber following a clinical assessment, full stop. No platform can legally supply it any other way. In practice, that means either your GP (though NHS access has strict phased criteria, discussed below), or a regulated private online pharmacy where a qualified prescriber reviews your case.

For a private prescription, the process at a properly regulated service looks like this: you complete a detailed health questionnaire, a GPhC-registered Independent Prescriber reviews it the same day, your identity is verified against photo ID, and your weight is confirmed on a live video call. Only then is a prescription issued and your medicine dispensed. That sequence protects you. It is also why buying Mounjaro online from a seller who skips any of those steps should be a red flag, not a selling point.

What about getting Mounjaro through the NHS if your BMI is under 30?

For most people with a BMI under 30, the NHS route is not currently available. NICE's appraisal of tirzepatide (TA1026) sets the NHS eligibility threshold at a BMI of 35 or above, combined with at least one qualifying weight-related condition, with phased rollout by the number of conditions present. That is not a moral judgement about who deserves treatment; it reflects how NHS commissioning is prioritised by need and budget. How do you know a Mounjaro seller is genuine?

The MHRA has issued repeated warnings about counterfeit weight-loss pens sold online, including fake tirzepatide pens. In October 2025 the agency raided the first illicit UK manufacturing site producing counterfeit tirzepatide pens; roughly 20 million doses of illegally traded weight-loss medicines have been seized in enforcement operations. Some counterfeit pens have contained entirely different substances. The risks are not theoretical.

Safe sourcing means three things: a pharmacy on the GPhC register (check it), a prescriber who actually reviews your consultation rather than auto-approving it, and medicine supplied through the licensed UK supply chain. Sellers offering these medicines without a clinical assessment, at prices far below the market, or pushing discount codes are operating outside that framework.
