How to Attach a Needle to Your Mounjaro Pen Correctly

What do you actually need before you start?

Before touching the pen, check three things. First, confirm the pen is at room temperature: injecting straight from the fridge is uncomfortable and can cause a lump under the skin. How do you attach the needle to the Mounjaro KwikPen without making a mistake?

The KwikPen has a threaded needle tip covered by a rubber seal. Take a new needle (still in its outer packaging) and peel the paper tab back to expose the needle unit without touching the needle itself. Hold the pen body in one hand, point the tip upward, then press the needle unit straight onto the tip. Twist clockwise. You'll feel it catch the thread almost immediately; keep turning until it stops moving. Firm, not forceful.

Now remove the outer needle cap: pull it straight off and set it aside, you'll need it at the end. Underneath sits the inner needle cap. Pull that off too and discard it. The needle is now exposed and ready. One thing our prescribers are asked most weeks: can you reuse a needle to save money? No. A used needle's tip is microscopically blunted, which makes the next injection noticeably more painful and increases the risk of lipohypertrophy, a hardening of the fat tissue that affects absorption. Change the needle every single time.

What happens during and immediately after the injection?

Choose your injection site: abdomen (at least 5cm from the navel), front of the thigh, or upper arm. Rotate between sites each week, the same spot repeatedly leads to tissue changes that can reduce how reliably the medicine is absorbed. Pinch a fold of skin if you're lean, dial to your prescribed dose, press the pen firmly against the skin at a 90-degree angle, and press the injection button slowly and steadily. Hold it in place for at least ten seconds after the click, this gives the full dose time to deliver. Releasing too early is the most common cause of a partial dose.

When you withdraw the pen, replace the outer needle cap carefully, use the cap rather than your fingers to avoid needlestick injury. If you notice redness or a small lump at the injection site, that's common and usually settles within hours. Rotating sites regularly is the best prevention. How does needle size affect the injection experience?

Shorter, finer needles (4mm, 32G is a popular choice) cause less discomfort for most people regardless of body size, clinical evidence does not support the idea that heavier patients need longer needles for subcutaneous injections.
