The NHS rolls out access in defined phases rather than opening all at once, which means the NICE recommendation sets the clinical ceiling but the practical threshold depends on which cohort is active. One detail that trips people up: a BMI of 35 alone is not enough under the NHS rollout. You also need qualifying comorbidities, and the number required varies by cohort. The NICE guidance also specifies that if someone loses less than five percent of their body weight after six months at the highest tolerated dose, continuing treatment should be reviewed. That clinical checkpoint exists for good reason, it keeps prescribing targeted at people who are responding.

For South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, BMI thresholds are set 2.5 kg/m² lower throughout the NHS rollout, reflecting established evidence that health risks appear at lower BMI readings in these groups.

The phased NHS rollout: which cohort is open now

The NHS is not making Mounjaro available to everyone who meets the NICE licence criteria at once. Access is being extended in steps, each one lowering the threshold slightly. Here is where the rollout stood as of summer 2026, drawing on NHS England's guidance on weight-management injections:

The first cohort (active since June 2025) covers adults with a BMI of 40 or above who have four or more of the five qualifying conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease and type 2 diabetes. The second cohort opened in June 2026, extending access to adults with a BMI between 35 and 39.9 who also carry four or more of those same conditions. A third cohort is expected around March 2027, which will include adults with a BMI of 40-plus and three qualifying conditions rather than four.

From April 2026, GP practices that have opted into the new contract arrangement can prescribe Mounjaro directly, without a specialist referral. Participation is voluntary, so whether your own GP surgery is involved depends on where you live. Practices that do take part are required to offer wraparound dietary and activity support alongside the prescription. If your practice hasn't joined yet, the specialist-service route remains open, though waiting lists can be long.

Do you qualify for Mounjaro on the NHS right now, an honest assessment

The honest answer is that most people asking this question won't meet the current NHS criteria, at least not yet. Cohort one and two between them cover a relatively narrow band of the eligible population, people with high BMI and multiple serious conditions.
