The scenario most people describe goes like this: the new pen arrives, the injection goes fine, and then twelve hours later the nausea sets in. If that's where you are right now, you're not alone and you're not doing anything wrong. It's an uncomfortable but normal response to a higher semaglutide dose, and it usually peaks around day two or three before backing off.
The most reliably useful thing you can do is eat less at each sitting, not skip meals altogether. Small portions of plain, lower-fat food (toast, rice, boiled potatoes, crackers) sit more easily than rich or fried meals. Wegovy slows gastric emptying, so heavy food lingers and makes nausea worse. Hydration matters more than people expect. Sipping water steadily through the day is better than drinking a large glass all at once when you're already queasy. Avoid alcohol and fizzy drinks on the days following a dose change. Sulphurous burps are one of the side effects people find most socially awkward, and they're caused by the same gastric-slowing mechanism behind the nausea. Food spends longer in the stomach, gas builds, and the result is frequent belching that can have an unpleasant smell. It tends to be worst in the first week after a dose increase.
Eating more slowly and chewing thoroughly reduces the air you swallow with food. Avoiding carbonated drinks, onions, eggs and high-fat dairy in the days after a dose step can also help. Some people find that eating their largest meal at lunchtime rather than in the evening (when digestion slows further overnight) makes the burping less persistent. Constipation and diarrhoea can both occur, sometimes alternating. Adequate fibre and fluid intake helps with constipation; for loose stools, plain starchy foods and electrolyte drinks are the practical first response. If diarrhoea is severe or leads to signs of dehydration (dizziness, dark urine, dry mouth) that warrants a call to your prescriber rather than self-management.
Tiredness is reported frequently in the early weeks, and it's often linked to eating less without adjusting the balance of what you eat. Protein adequacy is the thing most commonly underestimated: when overall calorie intake drops sharply, many people cut protein alongside fat and carbohydrate, which compounds the fatigue. Aiming to keep protein roughly steady (meat, fish, eggs, dairy, legumes) while the portions overall shrink is the practical fix.
Headaches in the first week or two often reflect mild dehydration or a change in eating patterns rather than a direct drug effect. More water, less caffeine, and consistent meal timing help. If headaches are severe, sudden in onset, or accompanied by visual disturbances, don't put them down to Wegovy without speaking to a clinician.
Redness, itching or a small lump at the injection site is common and usually settles within a day or two. Rotating sites (abdomen, thigh and upper arm) and allowing the pen to reach room temperature before injecting reduces local reactions for most people. Most side effects are unpleasant rather than dangerous, but there is a short list of symptoms that require prompt medical attention rather than dietary tweaks and patience.
Severe stomach pain that persists and spreads to your back, with or without vomiting, is the key one. In January 2026 the MHRA issued a Drug Safety Update specifically highlighting acute pancreatitis as a known, infrequent but serious risk with GLP-1 medicines. Other symptoms that warrant urgent contact with your prescriber or a healthcare professional include signs of a serious allergic reaction (facial swelling, difficulty breathing, a widespread rash), persistent vomiting where you can't keep fluids down, signs of significant dehydration, and any new or worsening symptoms of low mood or thoughts of self-harm. Sudden changes in vision should also be assessed promptly, particularly for anyone with diabetes.
