A widely repeated idea is that nausea on semaglutide is proof the medicine is doing something useful, a kind of badge of efficacy. That is not quite right. Nausea is a GI side effect driven by the way GLP-1 receptor agonists slow gastric emptying. It is common. It is not, on its own, a marker of how much weight you will lose, and pushing through it without any adjustments is not a clinical virtue. The more useful frame: nausea is a signal about pace and habits, not potency. Many people feel it most sharply after the first injection at a new dose level. If yours has not eased after two weeks, or if it is stopping you eating adequately, that warrants a conversation with your prescriber rather than stoic endurance. One thing our prescribers hear most weeks: patients who tried to manage nausea by eating less, only to find that under-eating made it worse. Counterintuitive, but consistent.
Most of the strategies that work are mundane, which is partly why people dismiss them before trying properly. Eat smaller portions, more slowly. The stomach empties more slowly on semaglutide, so a portion that felt normal before may now feel enormous 20 minutes in. Stopping a little earlier than feels natural is often enough to avoid nausea setting in.
Avoid rich or fatty foods, especially in the first few days after a new dose. High-fat meals require more gastric work; on slowed gastric emptying, that can tip nausea from mild to unpleasant. Plain foods (rice, toast, boiled potatoes) sit more easily. Hydration matters too: sip water throughout the day rather than drinking large amounts at once. Injection timing is worth experimenting with. Some people find injecting in the evening reduces the peak-nausea window to overnight hours when they are asleep. Others prefer mornings. There is no universal answer; trial over a few weeks with your prescriber's knowledge is reasonable. Avoid lying down immediately after eating, staying upright for an hour after meals can help. Over-the-counter remedies such as ginger products and peppermint are commonly used for nausea. They are not a treatment for semaglutide's mechanism, but some people find them helpful for the sensation. If you are considering any antiemetic medication, check with your prescriber or pharmacist first, especially for interactions.
Most Wegovy nausea is unpleasant, not dangerous. There are exceptions, and knowing them is the point of this section. Seek urgent medical attention if you experience severe, persistent abdominal pain, particularly pain that radiates towards your back, with or without vomiting. This can be a sign of acute pancreatitis. In January 2026, the MHRA issued a Drug Safety Update specifically highlighting pancreatitis as an infrequent but serious GI risk with GLP-1 medicines, infrequent means it does not happen often, but when it does, prompt attention matters.
Also seek help if nausea and vomiting are severe enough to prevent you keeping fluids down for more than 24 hours, dehydration and kidney strain are real risks in that scenario. Signs of an allergic reaction (swelling of the face or throat, difficulty breathing) require emergency care. If you notice significant low mood or thoughts of self-harm after starting Wegovy, contact your GP or NHS 111.

Persistent nausea that has not settled after two or three weeks at a dose is not something to manage silently. A prescriber has several options: slowing the titration schedule, pausing at the current dose for longer before any increase, or reviewing whether Wegovy remains the right choice given your response so far. Stopping the medicine abruptly without clinical guidance is rarely the right answer, but neither is continuing indefinitely with a side effect that is affecting your quality of life.
