Using the Wegovy multi-dose pen: what you need to decide before you inject

Deciding where and when to inject each week

Consistency matters more than perfection. Pick a day that fits your routine (a Monday morning, a Sunday evening) and stick to it. If you miss your usual time, the injection can be given up to five days late; after that, skip it and resume on your next scheduled day. Your prescriber and the Patient Information Leaflet carry the definitive guidance on missed doses, so treat anything here as orientation rather than instruction.

Where you inject shapes how well semaglutide is absorbed. The three licensed sites are the abdomen (avoiding the two inches around the navel), the front of the thigh, and the outer upper arm. Rotating within and between those zones prevents the small lumps or skin changes that repeated injections in one spot can cause. A useful habit: keep a simple mental note of last week's site and move to the next area in sequence.

One misconception worth setting aside gently: many people assume injecting into the upper arm is somehow less effective or more painful than the abdomen. The evidence does not support that. All three sites are clinically equivalent for absorption; comfort and convenience are the deciding factors. If you find one site easier, use it, just keep rotating within it.

Preparing the pen: the choices that affect every dose

Before each injection, take the pen from the fridge and let it reach room temperature for about 15–30 minutes. Injecting cold semaglutide is not dangerous, but it is noticeably more uncomfortable and there is no reason to do it unnecessarily.

Check the liquid in the pen window. Semaglutide solution should be clear and colourless to slightly yellow, with no particles floating in it. If it looks cloudy, discoloured or contains visible matter, do not use it, contact your pharmacy or prescriber. This check takes five seconds and is worth every one of them.

Attach a new needle for this injection only. Push the outer cap onto the needle port firmly, then remove the outer and inner needle caps. Never pre-attach a needle and store the pen that way; leaving a needle on between doses risks air entering the cartridge and the solution leaking or drying at the tip. Once the needle is on, you are ready to inject.

Clean the injection site with an alcohol swab and let it dry fully before inserting the needle, injecting through wet skin stings more and slightly increases infection risk. Pinch a fold of skin, insert the needle at a 90-degree angle, press the dose button until the counter clicks forward by one, then hold for six seconds before withdrawing. That six-second pause is the step most people skip on the first attempt; it ensures the full dose is delivered. Storing and disposing of the pen between doses

The multi-dose pen lives in the fridge between weekly injections. Keep it away from the freezer compartment and from direct contact with the cooling element at the back of the fridge shelf, freezing destroys the medicine. If the pen has accidentally frozen, do not use it.

Storage outside the fridge is sometimes necessary: travelling, staying somewhere without reliable refrigeration, or a power cut. The Patient Information Leaflet specifies the exact number of hours the pen can safely be kept at room temperature; because that window can differ between product batches and is subject to update, we do not quote a figure here. Needle disposal is non-negotiable. Used needles are clinical waste. Recap the outer cap, remove the needle from the pen and drop it into a sharps container immediately. Placing used needles in household bins creates a real risk of needlestick injury for refuse workers. Most GP surgeries and pharmacies accept full sharps containers for safe disposal.

When to pause and speak to your prescriber

Semaglutide's side-effect profile is well characterised. Nausea, loose stools, constipation and indigestion are the most commonly reported effects, particularly in the weeks after a dose increase. They are not trivial if you are experiencing them, but they do typically ease as your body adjusts. Staying well hydrated, eating smaller portions and avoiding very fatty or rich meals reduces the severity for most people.

Some symptoms need prompt medical attention rather than waiting to see if they pass. Severe stomach pain that radiates through to the back (with or without vomiting) can indicate pancreatitis, which the MHRA specifically flagged in a Drug Safety Update for GLP-1 medicines in January 2026 as a known, infrequent but serious risk. Seek medical help immediately for pain of that character. The same applies to signs of a serious allergic reaction: swelling of the face, throat or tongue, difficulty breathing, or a severe rash.
