Why 15 mg is the ceiling, not the target

Mounjaro's six-strength ladder exists because the body needs time to adapt to a dual GIP and GLP-1 receptor agonist. The 2.5 mg starting pen exists solely to let your system settle, it has no meaningful therapeutic dose at that level. From there, the licensed schedule moves upward in roughly four-week steps, with the prescriber deciding at each rung whether you are ready to go higher based on tolerability and progress. Reaching 15 mg in the fastest possible time would mean starting in month one and arriving at the top by month six. Many people never need to.

In SURMOUNT-1, the large phase-3 trial whose results were published in the New England Journal of Medicine, participants on the 15 mg dose achieved around 20–21% average body-weight reduction over 72 weeks. Those on 5 mg and 10 mg also produced clinically meaningful results. The top dose produced the best numbers on average, but averages conceal a lot of individual variation. Some patients reach their goals at 7.5 mg and stay there. What the prescriber weighs before moving to 15 mg

The decision to increase toward the Mounjaro maximum dosage hinges on two things running at the same time: how much weight you are losing and how well you are tolerating the current dose. If nausea, vomiting or other gastrointestinal side effects are still noticeable, a prescriber will usually recommend staying at the current strength for another four weeks rather than pushing up. Tolerance generally improves as the body adjusts.

NICE's appraisal of tirzepatide (TA1026) notes that if someone has not achieved at least 5% weight loss after six months at the highest tolerated tirzepatide dose, continuing treatment should be reviewed. This means that reaching 15 mg and finding it no more effective than 10 mg for a given patient is clinically useful information, not a failure. The prescriber uses it to reassess the overall plan.
