NICE recommended tirzepatide for weight management in adults in December 2024 (Technology Appraisal TA1026), published after clinical trial data showed average body-weight reductions of around 20–21% at the 15mg dose over 72 weeks in the SURMOUNT-1 study. That evidence base is the reason 15mg became the headline figure, it is the maximum licensed strength and the one most often discussed in terms of results.
On the cost side, Eli Lilly's September 2025 list-price increase changed the commercial landscape significantly. Before it, the 15mg pen carried a list price of around £122. After it, around £330. One thing NICE's appraisal also makes clear: 15mg is a maintenance dose, reached after progressive titration from 2.5mg. Most patients are not on it from day one. The prescriber decides pace and dose based on tolerability and clinical response, that process matters as much as the price of the pen itself.
Advertised per-pen prices frequently exclude several real costs. A consultation fee is common, some providers charge £20–£50 separately for the clinical assessment. Delivery charges, prescription fees and ongoing dose-review consultations are sometimes billed as add-ons. Aftercare (the clinical oversight that catches problems early, answers questions mid-month and reviews evidence before a dose increase) is often not mentioned at all until you need it.

The MHRA has warned repeatedly about counterfeit weight-loss pens circulating online, including fake tirzepatide pens seized from an illicit manufacturing site in the UK in October 2025. Approximately 20 million doses of illegally traded weight-loss medicines, worth around £45 million, have been seized by enforcement action. Fake pens have been found to contain insulin in some cases, a serious risk to anyone without diabetes. The MHRA's Yellow Card reporting scheme allows anyone to flag a suspect seller.
For a prescription-only medicine at the top of a titrated dosing schedule, the legitimate supply chain and clinical oversight are not optional extras bolted on to justify a higher price. They are what make the treatment safe to use. The right question is not which provider lists the lowest number, but which provider can demonstrate GPhC registration, genuine sourcing through the licensed UK supply chain, and a prescriber-led clinical process. Price is a relevant factor once those things are confirmed, not before.
