There's a dose left in my Mounjaro pen — here's what that means

You've finished your fourth dose, so why does the pen still look part-full?

Picture this: it's Sunday evening, your usual injection day. You press the button, hold for ten seconds, withdraw the needle, and glance at the pen. There's still a noticeable amount of liquid in there. The instinct is to wonder whether you did something wrong, or whether you've wasted part of your medicine.

You haven't. Mounjaro KwikPens are manufactured with a deliberate overfill, a buffer of fluid built into every cartridge to ensure that each of the four programmed doses is delivered in full, even accounting for small variations in the dispensing mechanism. The device releases a precise, fixed volume per activation. Once the fourth dose has been given, the pen reaches its end-of-use point and the mechanism locks. How to tell whether your pen genuinely has doses left, or has reached the end

The liquid level alone is a poor guide. The indicator window (the small viewing area on the side of the KwikPen) and the device's dose counter or locking behaviour are what actually tell you where you stand.

On a fresh pen, the first dose is available and the window shows the starting position. After each successful injection, the counter or indicator advances. When the fourth dose has been delivered, the KwikPen will not activate again: the plunger locks and the button will not depress fully. If your pen still activates and delivers medication, you have doses remaining. If it has locked, you don't, regardless of any fluid still visible.

Some people notice what looks like a fifth dose's worth of fluid and wonder whether the pen was overfilled beyond its labelled count. If you're genuinely uncertain whether a dose was delivered fully (perhaps the needle wasn't inserted properly, or you didn't hear or feel the expected click) the right call is to contact your prescriber before injecting again. Taking a double dose is not the answer. Your prescriber can walk you through what happened and advise on next steps based on your specific situation.

What to do with a pen once you've used all four doses

A finished KwikPen should go into a sharps bin, not a household bin or recycling. If you don't yet have a sharps container, your local pharmacy can usually supply one or advise where to get one; you can also pick one up from our sharps container if you'd like one sent with your next order.

Timing matters here, too. If your next pen is due to arrive on payday, a bank holiday Monday, or during a school holiday week when deliveries feel unpredictable, it's worth planning ahead so you're not left without medication on your injection day. Storage of your current pen matters just as much as the next one: Mounjaro pens should be kept refrigerated between 2°C and 8°C. When the leftover liquid question points to something bigger
