What the trial evidence tells us about why dupes can never replicate Mounjaro

Mounjaro's results in clinical trials rest on a very specific mechanism. Tirzepatide activates two gut-hormone receptors simultaneously (GIP and GLP-1) a combination that no other weight-loss medicine licensed in the UK currently shares. The SURMOUNT-1 trial, published in the New England Journal of Medicine, randomised 2,539 adults with obesity and reported average body-weight reductions of around 20–21% at the 15mg dose over 72 weeks. Those numbers are the product of a precisely manufactured, rigorously tested molecule, formulated at exact concentrations, delivered through a validated KwikPen at calibrated doses from 2.5mg upwards.

A dupe cannot replicate any of this. A product that claims to contain tirzepatide without MHRA approval has not been through the manufacturing, stability or clinical-testing standards that produced those trial results. The active ingredient might be absent, under-dosed, over-dosed, or something else entirely. You would have no way of knowing from the packaging alone.

This is not a theoretical concern. MHRA enforcement across 2024–2025 uncovered roughly 20 million doses of illegally traded weight-loss medicines (approximately £45 million street value), and raided the UK's first illicit manufacturing site producing counterfeit tirzepatide pens in Northampton in October 2025. Some seized fake pens contained insulin, a drug that in the wrong dose can cause life-threatening hypoglycaemia. Why the search for a dupe is understandable, and what a legitimate alternative looks like

A question our prescribers hear most weeks is some version of: "Is there a cheaper version I can get without jumping through all the hoops?" It is a fair question. Mounjaro is not cheap, and private prices have risen since Eli Lilly adjusted UK list prices in September 2025. The honest answer is that the hoops (the clinical assessment, the prescriber review, the identity checks) are the thing that keeps you safe, not an inconvenience layered on top of a straightforward purchase.

Wegovy (semaglutide) is a separate licensed weight-management injection with its own strong evidence base, and for some people it may be the more suitable choice clinically. How to tell a genuine Mounjaro supply from a counterfeit one

Genuine Mounjaro reaches UK patients through one route: a licensed pharmacy, dispensing against a valid prescription issued by a qualified prescriber, sourcing stock from Eli Lilly's authorised UK distribution network. That chain is auditable at every link. The MHRA licenses the medicine; the GPhC registers and inspects the pharmacy; the prescriber holds a professional registration you can verify; and the packaging carries batch numbers that can be traced.

Red flags that a product is not genuine include: being offered without a prescription or clinical assessment, pricing far below current UK market rates, sellers operating via social media direct messages or unofficial websites, and no verifiable pharmacy registration. You can check whether any online pharmacy is GPhC-registered at the GPhC register, a legitimate service will be listed there and will give you its registration number without hesitation.

If you have bought or been offered a product described as a Mounjaro or tirzepatide dupe, report it to the MHRA via the Yellow Card scheme. Do not use it.

Getting genuine Mounjaro through a regulated service
