Mounjaro for Diabetes on the NHS: Two Different Paths, One Decision

The diabetes route and the weight-management route are not interchangeable

This is the fork in the road that catches most people out. What NICE TA1026 actually says, and where NHS weight-management access stands in 2026

Even meeting the criteria on paper does not guarantee a prescription. NHS England requires that weight-management prescribing is accompanied by a structured diet and activity support programme, and individual GP practices opted into the new 2026/27 contract on a voluntary basis, so availability genuinely varies by postcode. Having type 2 diabetes counts as one of the five qualifying conditions for the weight-management NHS route. So if you have diabetes alongside three or four other listed conditions and your BMI is high enough, that may bring you within the current or upcoming NHS cohort for weight management, but you would still need to be referred and assessed through the proper pathway rather than self-selecting.

Who to speak to, and what's still uncertain

The honest answer about NHS diabetes prescribing is that local formulary decisions mean there is no single national rule governing whether your GP can offer tirzepatide for type 2 diabetes today. Some areas have it available; others don't yet. Your diabetes team or GP is the right first call, not a pharmacy, and not a search engine.
