Mounjaro injection technique: a step-by-step guide to injecting safely

What the official guidance says about subcutaneous injection technique

The SmPC and Patient Information Leaflet for Mounjaro, published on the electronic Medicines Compendium (eMC), describe the Mounjaro injection as subcutaneous, meaning it goes into the layer of fatty tissue just beneath the skin, not deep into muscle. This is the same delivery route used by insulin pens and is well-established in clinical practice. In the SURMOUNT-1 trial, which randomised 2,539 adults over 72 weeks, participants self-administered the weekly injection throughout, confirming this is designed to be done independently once technique is established.

The logic behind the subcutaneous route is that tirzepatide is absorbed steadily from the tissue depot over the week, keeping blood levels stable enough to sustain appetite regulation and the dual GIP/GLP-1 receptor effects. Rushing the injection or choosing the wrong site can affect that absorption.

A question our prescribers hear regularly is whether technique actually changes outcomes. The honest answer is that poor technique rarely affects weight loss dramatically, but it can cause more pain, localised bruising or lumpy skin over time, which then makes people reluctant to continue. Small habits make a real difference.

Choosing and rotating your injection site

There are three approved sites: the abdomen (at least a few centimetres away from the navel), the outer thigh and the upper arm. Injecting into the same spot week after week can cause lipohypertrophy, a build-up of thickened fatty tissue that feels firm, absorbs medicine unpredictably and can become permanently visible.

A practical system: keep a simple note on your phone of where you injected last. Many people cycle through abdomen left, abdomen right, left thigh, right thigh over four weeks, then start again. Some prefer the upper arm (easier with auto-inject pens, harder to self-administer manually). There is no single correct pattern, consistency and genuine rotation matter more than the sequence.

Avoid injecting into skin that is bruised, tender, scarred or affected by a rash. The injection itself: what to do, and what to avoid

The Mounjaro KwikPen is a pre-filled device, one pen contains four weekly doses. Before each use, take it from the fridge (many people keep it in the fridge door so it's easy to spot) and let it sit at room temperature for a few minutes; a cold injection stings more. Do not shake the pen.

Clean the injection site with an alcohol swab and let it dry fully before injecting, damp skin can sting. Sterile alcohol swabs are available separately if you need to restock. Hold the pen firmly against the skin, press the injection button and keep the pen in place until the dose counter returns to zero. This takes a few seconds, releasing early risks delivering an incomplete dose.

A brief note on timing: Mounjaro is injected once a week on the same day each week where possible, at any time of day, with or without food. If you need to shift the day, the Patient Information Leaflet explains the minimum gap required, defer to that, or ask your prescriber via aftercare.

Needles, storage and keeping your routine safe

Storage is non-negotiable. Pens must be kept refrigerated at 2–8°C before first use. Once in use, the pen can be stored at room temperature for a limited period, but the exact window is stated in the Patient Information Leaflet and that figure is the one to follow, not third-party sources. Exposing a pen to heat, freezing temperatures or direct sunlight can degrade the medicine inside without any visible change to the pen.
