Each pen comes with its needle already attached and concealed inside a safety shield. The needle is 4 mm in length and 32 gauge in diameter, making it one of the finer, shorter needles used in injectable weight-loss treatment. Your Mounjaro KwikPen arrives in plain, unbranded packaging via DPD, with a tracking notification so you know exactly when to expect it. Inside the outer carton is the pen itself, a pre-filled, single-use auto-injector. The cap covers a small orange tip, and the needle sits recessed inside that tip, shielded from view entirely. This is deliberate. The 32-gauge needle is finer than the needles used for many other injections and is short enough that most people cannot see it during the injection process, which tends to reduce anxiety at first use.
The 4 mm length is appropriate for subcutaneous injection across the three licensed sites (abdomen, outer thigh, and upper arm) in most adults, without needing to pinch the skin. Remove the cap, press the pen against clean skin, and press the button, that is the entire sequence from a needle-management perspective.
When you press the pen's base firmly against the injection site and activate it, the needle extends, delivers the full dose, and retracts in one motion. You hold the pen in place for the time stated in your Patient Information Leaflet to ensure the full dose is given. The needle never protrudes during storage or after the pen locks, the auto-retraction mechanism is part of why these pens are considered lower-risk for needle-stick injury compared with traditional syringe-and-needle combinations.
The 32-gauge diameter means the needle is very fine. Gauge numbers work inversely, the higher the number, the narrower the needle. A 32-gauge needle is thinner than the 25- or 27-gauge needles used in some older injectable medicines, which is a meaningful practical difference. Most people describe the sensation as a brief sting, if anything. Needle gauge and length are set by Eli Lilly for the KwikPen design. You cannot swap them out or use a different needle, the pen is not designed for that and attempting to modify it would compromise both safety and dose accuracy. Once the pen has delivered its dose and the needle has retracted and locked, the pen is finished. It is single-use. The locked needle tip means it should still be treated as a sharps item, do not put it in general household waste or a standard recycling bin. A 1-litre sharps container is the appropriate receptacle; most local councils and GP surgeries offer collection or disposal for sealed sharps boxes.
The pen holds four weekly doses, which means one pen lasts four weeks. Each actuation uses the same integrated needle, you are not loading a new needle for each weekly dose. The pen dispenses one dose per use and cannot be re-dosed or overfired. Keep it refrigerated between uses, following the storage guidance in your Patient Information Leaflet for the exact temperature range and any room-temperature window that applies.

Needle gauge and length are not cosmetic details. A needle that is too long for subcutaneous delivery risks hitting muscle, which changes absorption and increases discomfort. Too wide and the gauge contributes to unnecessary tissue trauma. The 4 mm / 32-gauge specification in the KwikPen is the result of clinical validation by Eli Lilly as part of the device's licensing, it is not interchangeable with standard pen needles from other systems.
