Using the Mounjaro KwikPen correctly: needle design, technique and safe disposal

The myth: Mounjaro uses attachable 4mm pen needles like some other injectables

If you have ever used insulin or older GLP-1 medicines, you may be used to screwing a separate 4mm or 8mm needle onto the pen cap before each dose. It is entirely understandable to search for replacement 4mm needles with that habit in mind, but the Mounjaro KwikPen works differently. Eli Lilly designed it as a fully self-contained device: the needle is built into the pen tip, shielded by a protective cover, and you never see or handle it directly. When you press the purple injection button, the needle extends, delivers the dose, and retracts, all in one movement.

This design is intentional. It removes the steps most likely to cause needle-stick injuries or dosing errors: attaching a needle at the wrong angle, forgetting to prime, or re-using a needle from a previous dose. The Mounjaro KwikPen was built so that the technique you need to learn is positioning and button-press only, not needle management. What actually goes into good injection technique with the KwikPen

Because the needle is hidden, the focus of good technique shifts to three things: choosing the right site, preparing the skin, and pressing the button fully. The licensed injection sites are the abdomen, the front or outer thigh, and the upper arm. Rotating between them (and between different spots within each site) reduces the risk of skin thickening or lumps forming over time. Skin preparation matters more than people expect. Cleaning the site with an alcohol swab and letting it dry fully before pressing the pen against it takes about thirty seconds and meaningfully reduces infection risk. The pen should be held flat and firm against the skin (not at an angle) and kept there until the injection window turns yellow, which confirms the full dose has been delivered. Rushing that last step is one of the more common reasons people find their dose has not fully activated.
