The SURMOUNT-1 trial, published in the New England Journal of Medicine, followed 2,539 adults with obesity over 72 weeks. Participants on 15mg tirzepatide lost an average of around 20–21% of body weight. That is not a marketing figure, it is the dataset that NICE reviewed before recommending tirzepatide in December 2024 (updated September 2025).
SURMOUNT-5, also published in the NEJM in 2025, ran a direct comparison against semaglutide 2.4mg. Tirzepatide produced greater average weight loss in that head-to-head across 72 weeks. The reason this matters for anyone searching for Mounjaro online is straightforward. Because the evidence base is large and positive, demand is high. High demand has fed a secondary market of unverified sellers, some of whom are operating outside the law. Knowing what genuine treatment looks like (and what it is supposed to deliver) makes it easier to spot what does not pass scrutiny. Treatment starts at 2.5mg; that first dose exists to settle your system, not yet to drive weight loss. Everything that follows is a prescriber's decision.
Mounjaro is a Prescription-Only Medicine under the Human Medicines Regulations 2012. The only legal way to obtain it is with a prescription issued by a clinician who has assessed your medical history. That is true whether you visit a GP surgery, a hospital clinic, or a regulated online pharmacy.

Treatment is delivered through GP practices enrolled in the new GP contract or specialist services. Criteria are strict and phased, and a qualifying BMI alone does not guarantee access, your GP's practice must be participating.

The MHRA has issued multiple warnings about fake weight-loss pens sold via social media and unverified online sellers. Counterfeit pens seized in the UK have been found to contain insulin rather than tirzepatide, a potentially life-threatening substitution. In October 2025, the first UK site manufacturing counterfeit tirzepatide pens was raided in Northampton. Enforcement across 2025 recovered roughly 20 million doses (~£45m street value) of illegally traded weight-loss medicines.
Red flags are consistent: offers made without a prescription, prices dramatically below the private market rate, sellers operating via social media DMs, and discount codes for Prescription-Only Medicines. A legitimate seller will never offer a POM without a clinical assessment, will be findable on the GPhC register, and will not advertise medicine price cuts. If a deal looks too clean to be real, the MHRA advises reporting suspect sellers at Yellow Card.
