Your Mounjaro Pen Won't Twist — Here's Why That Happens

The big misconception: twisting shouldn't happen without the pen on your skin

Most people who contact us about a pen that won't twist have skipped one step without realising it. The Mounjaro KwikPen is not like a syringe where you set the dose in mid-air. The mechanism is pressure-activated: you press the pen firmly against the injection site first, and only then does the button engage to let you twist and deliver the dose. If you are sitting at a table trying to rotate the button with no skin contact, it will not move. That is not a fault.

Before anything else, check the needle cap is fully removed. The outer cap and the inner needle cap are two separate pieces, and the inner one is easy to miss, especially on your first few injections. One practical checking habit worth making before every injection: with the pen uncapped and held upright, look at the dose window. You should see the dose number clearly. If the window looks blank or you see a red indicator, the pen may be empty or not set correctly, and that tells you what to investigate before pressing it against your skin at all. When the pen genuinely won't turn, and what to check

If you have confirmed the needle caps are off, you are pressing the pen firmly and evenly against a clean injection site, and the button still will not twist, there are a handful of real causes to work through. Cold is the most common one. A pen taken directly from the fridge and used immediately can feel stiff throughout, the mechanism is not seized, it just needs to warm up. Leave it at room temperature for up to 30 minutes before trying again. The second thing to check is whether the pen is already empty. Each KwikPen contains four weekly doses. After the fourth injection the pen locks permanently, it will not reset, and it is not meant to. A third possibility is that the pen sustained damage in transit or storage, a crack in the casing, a jammed internal mechanism, or a pen that was frozen and thawed, which can compromise the device. Some people find the pen will rotate a little but not reach the click that confirms the dose has been delivered. This usually points to one of two things: the pen is not held perpendicular to the skin (angling it can reduce the pressure needed to trigger the mechanism), or it is being lifted away from the skin before the full dose is delivered. The guidance is to hold the pen in place for a slow count of ten seconds after the click, keeping steady pressure throughout.

If the button turns but you see no medication expelled and the dose window does not change, the needle may be blocked or was not attached correctly.
