Trying to get Mounjaro through your GP? Here's the reality

You've asked your GP and hit a wall. Here's why.

Picture a common scenario: you've read about Mounjaro, your BMI qualifies on paper, and you raise it with your GP. They're sympathetic but can't prescribe it. This isn't obstruction, it's the current shape of NHS commissioning. Under NICE technology appraisal TA1026, which governs NHS prescribing of tirzepatide, the initial qualifying cohort requires a BMI of 40 or above plus at least four of five specific conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, and type 2 diabetes. That is a narrow group.

From June 2026 a second cohort opened (BMI 35 to 39.9 with four or more of those same conditions) but the circle is still tight. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance, typically 2.5 kg/m² below the published figures. Even then, every NHS patient must be enrolled in a structured diet and activity programme alongside medication. Meeting the criteria on paper doesn't mean your practice is participating: GP involvement is optional under the 2026 contract, so provision is genuinely patchy. From around March 2027, NHS eligibility is expected to broaden again, BMI 40 or above with three qualifying conditions rather than four. But that's months away, and waiting lists will grow as each cohort opens up. If you don't currently meet the NHS threshold, that's not a clinical opinion about whether treatment would help you. It's a commissioning decision.

What a GP can and can't do privately

Some GP practices offer private prescribing. A GP working privately holds the same prescribing authority as any other independent prescriber and can legally assess and prescribe Mounjaro for weight management. The medicine's licence covers adults with a BMI of 30 or above, or 27 or above where at least one weight-related condition is present, a considerably wider group than the NHS cohorts described above.

In practice, not many GP practices have set up private weight-loss prescribing pathways. Those that have often charge separately for the consultation, the private prescription, the medicine itself, and sometimes a dispensing or delivery fee. One thing worth knowing: whichever prescriber you use, the treatment itself is the same medicine from the same licensed UK supply chain. The private pharmacy route as an alternative

Side effects and safety: the same conversation wherever you prescribe

Any prescriber, NHS or private, should talk through the side-effect profile before starting treatment. The most common effects are gastrointestinal: nausea, diarrhoea, constipation, and sometimes a persistent burping or reflux sensation. These tend to peak when you first start or after a dose increase, and most people find they settle within a week or two. More serious but less common risks include pancreatitis.
