You've read that Mounjaro produces dramatic results and you want to know if the NHS will prescribe it for you. The honest answer is: it depends on more than your BMI, and the criteria are changing in phases.

NICE recommended tirzepatide for NHS use in December 2024 (updated September 2025). The current rollout, which began in June 2025, covers adults with a BMI of 40 or above who also have four or more of five specific weight-related conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes. From around June 2026, the second cohort opens to BMI 35–39.9 with the same four-condition requirement. Lower BMI thresholds (2.5 kg/m² less) apply for South Asian, Chinese, Middle Eastern, Black African, and African-Caribbean backgrounds.

Meeting those numbers is necessary but not sufficient. Every NHS patient must also take part in diet and activity wraparound support. And in practice, access depends on whether your GP practice has opted in, or whether your local integrated care board has activated the pathway. What the trial figures really mean when you're trying to estimate your own result

There's no personalised Mounjaro calculator that can tell you how much weight you'll lose, because weight loss on tirzepatide varies meaningfully between individuals. What clinical trials give you is a distribution, and the headline numbers are averages across thousands of participants.

In SURMOUNT-1 (a 72-week trial involving 2,539 adults with obesity) participants on the 15 mg dose saw an average body-weight reduction of around 20–21%. That's a population average. Some people lost more; some less. The SURMOUNT-5 trial, published in the New England Journal of Medicine in 2025, compared tirzepatide directly with semaglutide 2.4 mg and found greater average weight reduction with tirzepatide over 72 weeks. If you want to use those figures as a rough sense-check, take your current weight, apply 15% and then 20%, and you have a reasonable bracket to think about. But the prescriber's job is to assess whether you're likely to tolerate and benefit from treatment, not to promise an outcome. No calculator can replace that, and no responsible service should.

Treatment starts at 2.5 mg (a dose whose job is to let your system adjust, not to produce weight loss) and titrates upward over months under prescriber guidance. Keeping the pen in the fridge door and injecting on the same day each week is the kind of habit that makes a real difference to consistency.
