Getting the Mounjaro weight loss jab on the NHS: a plain-English guide

Step 1, Find out which NHS cohort you fall into (if any)

NICE recommended tirzepatide for NHS use in December 2024 (Technology Appraisal TA1026), but it attached conditions. The rollout is phased by BMI and the number of qualifying comorbidities a patient carries. For South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, the BMI thresholds are reduced by 2.5 kg/m².

Meeting those numbers on paper is a starting point, not a guarantee. Every NHS patient must also take part in wraparound diet and activity support, and your GP practice has to be participating in the scheme, participation is optional under the 2026/27 GP contract. Step 2, If you don't qualify (or don't want to wait), understand the private route

The NHS route suits some people and not others. Waiting lists at specialist weight management services exist; GP participation in the new contract is patchy; and many people with a BMI of, say, 32 and hypertension simply won't qualify under current NHS thresholds. Step 3, Go through a proper clinical assessment, wherever you access it

Whether your prescription comes via an NHS specialist service or a private GPhC-registered pharmacy, the legal requirement is the same: a prescriber assesses you before anything is dispensed. What the clinical evidence shows, and what it doesn't promise

Mounjaro's NHS recommendation rests on strong trial data. In the SURMOUNT-1 study, published in the New England Journal of Medicine, adults at the 15mg maintenance dose lost around 20–21% of body weight on average over 72 weeks, alongside a reduced-calorie diet and increased activity. In the SURMOUNT-5 head-to-head trial, tirzepatide produced a greater average reduction than semaglutide 2.4mg. These are population averages from controlled trials, individual results vary, and no outcome is guaranteed.

Side effects are mostly gastrointestinal: nausea, loose stools, constipation, indigestion. They tend to be most noticeable when you start or after a dose increase, and often settle within a couple of weeks.
