Step 1: Understand the NICE-set BMI floor for NHS Wegovy

For people from South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, NICE applies BMI thresholds that are roughly 2.5 kg/m² lower, so a threshold of 32.5 rather than 35 in the standard band. This reflects well-established evidence that metabolic risk appears at lower BMI values in these groups. Step 2: Know that the NHS route runs through specialist services only

Even if your BMI clears the NICE threshold, NHS access to Wegovy is not a straightforward GP conversation. NICE's TA875 recommendation specifies that semaglutide should be used within a specialist weight management service, as part of a multidisciplinary package that includes dietary support, physical activity guidance and psychological input. This is not a box-tick requirement, it shapes where and how the medicine is prescribed.

In practice, that means a referral from your GP to a tier 3 or tier 4 obesity service, and then a wait. Waiting times vary considerably across England, and in many areas they are measured in months. Scotland, Wales and Northern Ireland each have their own commissioning arrangements, so criteria can differ.
