Step 1: the morning routine that makes it work

Step 2: the dose escalation your prescriber manages

Novo Nordisk's oral semaglutide for weight loss follows a four-rung schedule: 1.5 mg, 4 mg, 9 mg, then 25 mg. Each rung lasts a minimum of one month before the next increase, the pacing is there to give the body time to adapt and to keep side effects manageable.

The 25 mg tablet is the maintenance dose. For anyone already established on the 2.4 mg weekly Wegovy injection, a prescriber may consider moving directly to the 25 mg tablet, though that transition is a clinical decision made on a case-by-case basis, not a standard rule.

It is worth being clear about what these strength numbers mean in context. The 1.5 mg, 4 mg, 9 mg and 25 mg figures are specific to the Wegovy weight-loss tablet. They are not the same as Rybelsus, also oral semaglutide from Novo Nordisk, but licensed for type 2 diabetes at different strengths (3 mg, 7 mg, 14 mg) and a different clinical purpose. Step 3: what the trial evidence actually showed

The OASIS 4 phase-3 trial enrolled 307 adults with obesity or overweight plus at least one weight-related condition, without diabetes, over 64 weeks. Alongside lifestyle changes, participants taking the Novo Nordisk oral Wegovy achieved an average weight loss of around 13.6% compared with around 2.4% on placebo. Among those who adhered fully to treatment, the average reached approximately 17%.

That 17% figure needs its context kept intact: it describes a fully-adherent subgroup, not the trial average. Both numbers are meaningful, but they answer different questions. The most common side effects in OASIS 4 were gastrointestinal (nausea, diarrhoea, constipation and related symptoms) reported in about 74% of the semaglutide group versus 42% on placebo. Most were mild to moderate and tended to ease as the body adjusted, particularly after dose increases settled. NHS status, eligibility, and what comes next

The Wegovy tablet has no NICE appraisal yet. Until one is completed and published, it is not available on the NHS, private prescription is the only route. A NICE appraisal takes time; the injection form of Wegovy was approved by MHRA years before NICE recommended it for NHS use under TA875.

The licensed eligibility criteria mirror those of the injection: adults with a BMI of 30 or above, or 27 to 29.9 with at least one weight-related condition such as high blood pressure, high cholesterol or obstructive sleep apnoea. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. BMI alone does not determine suitability; a prescriber weighs the full clinical picture, including any contraindications or medicines that may interact.

The tablet is not licensed for people under 18, and it is not recommended during pregnancy, while breastfeeding, or for anyone trying to conceive. The MHRA's approval is specifically for the weight-management indication; Novo Nordisk's oral semaglutide is not approved for the cardiovascular or liver-disease indications that the injection now holds in the UK.
