What the current evidence says about oral tirzepatide

Eli Lilly has been running phase 3 clinical trials on an oral form of tirzepatide, the programme is real and the results have been watched closely. Data published in 2024 showed meaningful weight reduction at higher oral doses, with tolerability profiles broadly consistent with the injection. That research matters, but it has not yet translated into UK regulatory approval. The MHRA licenses medicines based on its own review of the evidence, and as of summer 2026 no application for an oral tirzepatide weight-loss product has received authorisation in the UK. Until it does, any product sold as an oral Mounjaro or tirzepatide tablet in the UK sits outside the licensed supply chain entirely, which carries serious safety implications covered below.

For context on how tirzepatide's injection form performs, the SURMOUNT-1 trial published in the New England Journal of Medicine reported an average body-weight reduction of around 20–21% at the 15 mg dose over 72 weeks. That's the benchmark any oral version would need to approach. Whether an oral formulation reaches equivalent absorption and efficacy is a pharmacology question the ongoing trials are still answering.

The broader tirzepatide clinical programme has enrolled more than 10,000 participants across the SURMOUNT and SURPASS series combined, making it one of the most studied obesity medicines to date. An oral version would need its own body of evidence before UK prescribers could use it.

The oral GLP-1 that is licensed for weight loss in the UK right now

The reason this question is live is partly because oral GLP-1 treatment is no longer theoretical. On 11 June 2026 the MHRA approved Wegovy tablets (oral semaglutide co-formulated with an absorption enhancer called SNAC) making the UK the first country in Europe to license an oral GLP-1 medicine specifically for weight management. The difference between an oral tirzepatide and oral semaglutide matters clinically: tirzepatide is a dual GIP/GLP-1 agonist, semaglutide a GLP-1 agonist only, and the two drugs have distinct trial profiles.

The Wegovy tablet maintenance dose is 25mg once daily, reached by titrating through 1.5mg, 4mg and 9mg over several months. It's taken first thing in the morning, on an empty stomach, with a small amount of plain water, and you wait at least 30 minutes before eating, drinking or taking other oral medicines. Unlike the injection, it needs no refrigeration, which some people find genuinely practical. The OASIS 4 phase 3 trial, cited in the MHRA's approval announcement, reported around 13.6% average weight loss over 64 weeks versus around 2.4% with placebo. Among participants who adhered fully to the protocol, the figure was closer to 17%. Those are solid numbers, though they sit below tirzepatide's injection-form results at higher doses.

Why buying unlicensed 'oral Mounjaro' online is a different risk category

The MHRA has been direct about counterfeit and unlicensed weight-loss medicines circulating online. In October 2025 the agency raided the first illicit UK manufacturing site producing counterfeit tirzepatide pens, seizing around 20 million doses with an estimated street value of £45 million. Unlicensed oral tirzepatide products follow the same pattern: no regulated supply chain, no verified active ingredient, no clinical oversight. The MHRA's Yellow Card scheme lets anyone report a suspected counterfeit or unsafe seller, and the agency asks people to use it.

A prescription-only medicine sold without clinical assessment is a red flag, full stop.
