Who can be prescribed semaglutide tablets, and what does the licensing actually cover?

The UK licence covers adults with a BMI of 30 or above, or a BMI of 27 to 29.9 alongside at least one weight-related condition such as high blood pressure, high cholesterol, prediabetes or obstructive sleep apnoea. Treatment is taken alongside a reduced-calorie diet and increased physical activity, the tablet supports those changes, it does not replace them.

There is one important boundary worth knowing before you read further: Wegovy tablets are not the same product as Rybelsus. Rybelsus is oral semaglutide licensed for type 2 diabetes, in 3 mg, 7 mg and 14 mg strengths. Those doses are not steps on the Wegovy weight-loss ladder and should not be described as such. What does the clinical trial evidence say about how much weight people lose?

The pivotal evidence comes from OASIS 4, a phase-3 trial of 307 adults with obesity or overweight plus at least one weight-related condition, without diabetes, over 64 weeks. Participants taking the tablet alongside lifestyle changes lost an average of around 13.6% of their body weight compared with around 2.4% for placebo. Among those who adhered fully to treatment, average weight loss reached around 17%. That second figure is sometimes quoted without its context, so it is worth being precise: 17% reflects full adherence, not the overall trial average.

For comparison, the weekly semaglutide injection at 2.4 mg produced around 15% average weight loss over 68 weeks in the STEP 1 trial, as reported by the New England Journal of Medicine. The tablet's 13.6% average sits slightly below that, which is broadly expected given the differences in bioavailability between an injection and an oral route. Whether the injection or the tablet is more suitable for a given individual is a clinical question, not simply a numbers question.

The most common side effects in OASIS 4 were gastrointestinal: nausea, diarrhoea, vomiting and constipation were reported by a higher proportion of tablet users than placebo. These effects were generally mild to moderate and tended to ease over time, particularly as the body adjusted to each dose step.
