What SURMOUNT-1 actually found, and what it didn't measure

SURMOUNT-1 is the trial most cited when people ask about tirzepatide weight-loss results. Published in the New England Journal of Medicine, it randomised 2,539 adults with obesity (without type 2 diabetes) to tirzepatide at 5mg, 10mg, or 15mg, or to placebo, over 72 weeks. Average weight loss at the 15mg dose was around 20–21% of body weight, roughly 22kg for someone starting at 110kg. Around 90% of participants at the highest dose lost at least 5% of their body weight, and around half lost 20% or more.

Those are group averages. Individual results in the trial ranged considerably, and the people who lost the most tended to be those who combined treatment with meaningful changes to diet and activity, exactly what the licence requires. Tirzepatide is approved alongside a reduced-calorie diet and increased physical activity, not instead of them.

The trial also ran for 72 weeks, which matters. Many of the most striking before-and-after changes occurred between months four and twelve, not in the first two or three. This is worth keeping in mind when reading social-media accounts that show dramatic results very early, they may be genuine, but they're not the median experience.

Changes beyond the scale: what before-and-after really captures

Weight in kilograms is the metric most people track, but it doesn't capture everything that changes. People in the SURMOUNT programme reported improvements in waist circumference, blood pressure, cholesterol levels, and markers of blood-sugar regulation, changes that don't photograph easily but matter enormously for long-term health.
