Wegovy 0.5mg side effects: separating the normal from the serious

The 0.5mg step is the second rung of a gradual titration — your system has had four weeks on 0.25mg to adjust, but nausea and digestive symptoms often resurface briefly at each increase.
Severe, persistent stomach pain that spreads to your back is not a normal side effect, it warrants urgent medical assessment because pancreatitis, though infrequent, is a known risk with GLP-1 medicines.
Most common side effects (nausea, loose stools, constipation, indigestion) are self-limiting; they typically ease within one to two weeks of settling at a new dose.
You can report any unexpected or serious side effects to the MHRA via the Yellow Card scheme at yellowcard.mhra.gov.uk, and your prescriber should know about anything that concerns you.

When you move up to the 0.5mg dose of Wegovy (semaglutide), side effects tend to peak before settling down — most are digestive, most are temporary, and most are manageable with a few straightforward habits. That said, a small number of symptoms do need prompt medical attention, and knowing which is which matters. Wegovy is a once-weekly prescription injection licensed in the UK for weight management; a prescriber assesses your suitability before any dose is issued.

What 0.5mg Wegovy side effects actually look like, and what to do about them

The biggest myth: side effects at 0.5mg mean Wegovy isn't working for you

A lot of people assume that if they feel rough after stepping up to 0.5mg, something has gone wrong. In fact, the opposite framing is closer to the truth. Nausea, queasiness and changes in bowel habit at this stage are a sign that semaglutide is doing what it's designed to do, slowing how quickly your stomach empties and adjusting gut-hormone signalling. The discomfort isn't a malfunction; it's your digestive system recalibrating to a higher dose.

That doesn't make it pleasant, of course. What it does mean is that riding out the first week or two at 0.5mg with some practical adjustments is usually enough. Eating smaller portions, choosing low-fat foods and avoiding lying down straight after a meal are the habits most people find genuinely helpful. One quick check worth doing: pour yourself a large glass of water and drink it slowly. Dehydration makes nausea meaningfully worse on GLP-1 treatment, and it's easy to forget to drink enough when your appetite has dropped sharply. If you want a closer look at what to expect specifically at this stage, our guide to Wegovy 0.5mg side effects goes through each one in detail. The full Wegovy side effects picture covers how each symptom tends to behave across the whole titration.

The NHS medicines guidance for semaglutide confirms that gastrointestinal effects are the most commonly reported, and that they are generally mild to moderate in severity and tend to resolve on their own as treatment continues. NHS guidance on semaglutide sets out the full list of known effects and what to do about each.

The side effects most people notice at 0.5mg, and the ones that take people by surprise

Nausea is the one almost everyone has heard about, and it is genuinely the most common. Vomiting happens too, though less often than nausea alone. Diarrhoea and constipation can alternate (occasionally in the same week, which is disorienting) and reflux or belching is a frequent complaint that nobody warns you about in advance. Fatigue and mild headaches crop up as well, particularly in the first few days after a new injection.

What surprises people more is the speed of appetite change. At 0.5mg, food can feel unappealing from the moment you inject, and that sudden drop in hunger sometimes comes with light-headedness if you forget to eat enough. Protein intake in particular tends to fall when appetite shrinks, and keeping it adequate matters for preserving muscle, your prescriber or a dietitian can help you work out practical ways to hit a reasonable target.

Injection-site reactions (redness, a small lump, mild bruising) are generally minor and rotate out quickly if you change the site each week. Abdomen, thigh and upper arm are all suitable. Some people also report low mood or difficulty sleeping in the first weeks; these are listed effects and worth mentioning to your prescriber if they persist. Our page on side effects from Wegovy explains how these symptoms are categorised and what the prescribing data says about how often they occur. For a detailed breakdown of how long individual symptoms tend to run, how long Wegovy side effects last covers the typical timeline.

There's also a small group of people who notice very little at 0.5mg. That's covered in its own right, experiencing no side effects on Wegovy explains why that happens and what it means.

When to get medical help, the symptoms that need urgent attention

Most side effects at 0.5mg are uncomfortable rather than dangerous. A few are not, and it is worth being clear about which category they fall into.

Pancreatitis is a known but infrequent risk with GLP-1 medicines. In January 2026 the MHRA issued a Drug Safety Update reminding prescribers and patients about this: severe stomach pain that does not ease, especially if it radiates towards your back and is accompanied by vomiting, needs same-day medical assessment. Do not wait to see if it settles. Call your GP or 111, or go to A&E if the pain is severe. The MHRA's Yellow Card scheme is the right place to report it afterwards as a suspected side effect.

Other symptoms that warrant prompt contact with a clinician: signs of a serious allergic reaction (swelling of the face, lips or throat, difficulty breathing, hives), gallbladder pain (sharp pain in the upper right abdomen, sometimes with fever or jaundice), significant dehydration from repeated vomiting or diarrhoea, and any sudden changes in mood, thoughts of self-harm, or suicidal ideation, these are listed in the prescribing information and must not be dismissed.

If you are unsure whether something needs attention, our prescribers are available through our aftercare seven days a week. You can also reach the team via our support page.

How 0.5mg fits into the wider Wegovy schedule, and what comes next

The 0.5mg dose is not where treatment stays. It follows four weeks on 0.25mg, which is purely a starter dose, its job is settling your system before any meaningful weight-management effect begins. After four weeks at 0.5mg, the titration continues upward in stages, with 1.0mg, 1.7mg, 2.4mg and, for eligible patients, 7.2mg as the licensed maintenance doses. Each step carries a similar likelihood of brief GI disruption, though most people find it less pronounced as the titration progresses and their body adjusts to the mechanism.

Because Wegovy is a prescription-only medicine, your prescriber reviews your progress before each dose change. At nume, that review happens before every repeat order, a real clinician checks your records, not an automated system. Our clinical team leads that oversight. If side effects at 0.5mg are persistent or severe enough to be affecting your daily life, your prescriber may recommend staying at the current dose for longer before progressing, rather than pushing through unnecessarily.
