Wegovy (semaglutide 2.4mg injection, made by Novo Nordisk) received its UK marketing authorisation for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 29.9 where at least one weight-related comorbidity is present. The conditions that can satisfy that second criterion include type 2 diabetes, hypertension, dyslipidaemia and obstructive sleep apnoea, among others. Below BMI 27, Wegovy has no licensed weight-management indication in the UK. That is not a policy decision made by any individual pharmacy or clinic, it reflects what the medicine was tested and authorised for, as set out in its Summary of Product Characteristics published on the electronic Medicines Compendium.
Some people arrive at this question because they have lost weight since starting treatment and their BMI has dropped. That is a separate clinical situation and one your prescriber should review directly, it does not automatically mean treatment stops, but continuing would be an off-licence decision requiring a proper clinical rationale.
It is worth being honest about why this matters: the licensing framework exists because semaglutide carries real pharmacological effects and a known side-effect profile. Using it at weights where the benefit-to-risk balance has not been formally established is not something any responsible UK prescriber will do without strong clinical grounds.
If your BMI sits in the 27–29.9 range, the route to Wegovy is still open, provided your health history includes at least one qualifying condition. This is where the assessment becomes genuinely clinical rather than numerical.
High blood pressure is one of the most common qualifying conditions at this BMI level; so is a raised cholesterol profile or pre-diabetes. For adults of South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, UK guidance reduces the standard BMI thresholds by 2.5 kg/m². One thing our prescribers are asked fairly often: can a GP prescribe Wegovy off-licence if BMI is under 27? Technically a doctor may prescribe off-licence, but it must be clinically justified, documented and explained to the patient, and the NHS does not fund it in that context. Most UK clinicians will not do so for weight management alone at a BMI below 27.
If you fall below the BMI threshold for Wegovy, there are a few honest directions worth considering. First, check your BMI accurately, the NHS BMI calculator is a reliable starting point, and BMI readings can shift meaningfully depending on when and how they are taken. If you are close to 27 and have a relevant health condition, a consultation is still worth pursuing.
Second, consider whether another licensed treatment fits better. Mounjaro (tirzepatide) carries the same BMI thresholds (≥30, or ≥27 with a comorbidity) so it does not open up access for those below 27 either. Third, if your BMI is genuinely below 27 and none of the qualifying conditions apply, the honest answer is that Wegovy is not the right path right now. Lifestyle support, dietitian input and a conversation with your GP about underlying metabolic health are sensible starting points. That is not the answer most people searching this query want to read, we know that. But it is the correct one, and it is better you hear it plainly than discover it after a wasted consultation.

On the NHS, Wegovy is available only through specialist weight management services under NICE technology appraisal TA875, which sets stricter criteria than the licence alone, including a BMI of 35 or above in most cases, at least one comorbidity and multidisciplinary support. The NICE guidance on semaglutide (TA875) sets this out in full. For the vast majority of people with a BMI under 30, NHS access is not a realistic near-term option.
