Start journey Learn moreTiredness is one of the more common side effects people notice in the early weeks of Wegovy treatment. Fatigue on semaglutide tends to be most noticeable when you start or move up a dose, and for most people it settles within one to two weeks as the body adjusts. That pattern is well-documented on the NHS semaglutide medicines page, which lists fatigue among the common effects alongside the better-known gastrointestinal symptoms. Wegovy is a prescription-only medicine, so any concerns about how you're feeling should be raised with your prescribing clinician rather than managed in isolation.

Picture the scene: it's day five on your first Wegovy pen, you've eaten less than you have in years, and by 3pm you're flagging badly. You wonder if something is wrong. Most likely, nothing is, but understanding the cause helps you respond to it correctly.
Semaglutide slows gastric emptying and dials down appetite through GLP-1 receptors in the gut and brain. That's the mechanism behind the weight loss, and it's also why some people feel low on energy early on. Your calorie intake may have dropped significantly before your body has fully adapted to running on less. Add in mild nausea, disturbed sleep, or the general effort of adjustment, and tiredness makes sense.
The full profile of Wegovy side effects covers this in more detail, but the short version is: GI symptoms and fatigue tend to peak together, and they tend to ease together. Most people who experience this find it is dose-specific, it reappears briefly after each step up and then fades. Our clinical page on whether Wegovy causes fatigue explains the physiology more fully if you'd like to dig in.
Before calling anyone, run through this quickly: fill a large glass of water and drink it. Dehydration is surprisingly easy to miss when nausea reduces your thirst, and it compounds tiredness significantly. That one check takes under a minute and often reveals the simplest fix.
Beyond hydration, the factors most commonly underlying Wegovy-related fatigue are: insufficient protein (aim to keep protein at each meal even when appetite is low), disrupted sleep from nausea overnight, under-eating to the point of running on near zero, and a sudden drop in caffeine intake if you've gone off coffee. None of these require a prescription change, they require a practical adjustment.
The fatigue and semaglutide guide covers specific dietary strategies in more depth. Gentle movement (even a ten-minute walk) can also help; it improves circulation and mood without burning energy you don't have. Avoid the temptation to cut calories further to speed up results while you're already tired. That approach tends to backfire, making fatigue worse and making it harder to sustain treatment long-term. If you're genuinely unsure how much you should be eating alongside Wegovy, our prescribers can discuss this with you.
Tiredness that is mild and comes and goes in the first few weeks is expected and manageable. The picture that requires same-day help is different: extreme, sudden exhaustion that stops you functioning, fatigue combined with severe stomach pain (especially pain radiating through to your back), a racing or irregular heartbeat, breathlessness, confusion, or signs of serious dehydration such as no urine output for many hours.
Severe stomach pain is flagged specifically because, in January 2026, the MHRA issued a Drug Safety Update on GLP-1 medicines highlighting acute pancreatitis, a serious but infrequent risk. The symptom to act on is intense, persistent abdominal pain that reaches the back, with or without vomiting. If that describes what you're feeling, stop treatment and call 111 or go to A&E. Do not wait for your next review appointment.
For less urgent but persistent fatigue that is affecting your daily life after more than two or three weeks at a stable dose, the right step is a conversation with your prescriber about whether a slower titration schedule might suit you better. You can also explore the typical duration of fatigue on Wegovy to calibrate what's within normal range. Suspected side effects can be reported to the MHRA via the Yellow Card scheme, this reporting genuinely helps regulators track safety signals across the full patient population.
A lot of people starting Wegovy feel they have nowhere to turn between prescription reviews. That's not how things work here. nume's priority aftercare runs seven days a week, and you can reach the team through our contact page any time you have a concern between orders.
Our clinical team, overseen by our lead prescriber, reviews consultations personally, not through automated triage. If fatigue is affecting your quality of life or you're unsure whether what you're experiencing is within normal range, a prescriber can assess whether your dose schedule needs adjusting, whether something else may be contributing, or whether a pause is clinically appropriate. That's the kind of clinical oversight that matters with a prescription medicine. You can read more about how nume operates and what we prioritise on our about us page.
If you're not yet on treatment and are weighing up whether the side-effect profile of semaglutide is right for you, the semaglutide fatigue side effects page sets out the evidence clearly, and starting a free consultation through our weight-loss treatments page lets our prescribers discuss suitability with you directly. They cover side effects, your medical history, and what to expect, that conversation happens before any prescription is written.
