Does the NHS prescribe Wegovy, and who actually gets it?

This is a question our prescribers hear most weeks, and the short answer is: yes, but the conditions attached are significant. NICE recommended semaglutide for weight management in 2023 under guidance TA875, but only within specialist weight management services that provide full multidisciplinary care, not as a standard GP prescription. For most patients, that means a formal referral to a Tier 3 or Tier 4 service, which in many parts of England involves a wait measured in months rather than weeks.

To be considered under the NHS pathway, adults generally need a BMI of 35 or above and at least one weight-related condition such as high blood pressure, type 2 diabetes, or obstructive sleep apnoea. NICE's thresholds are set 2.5 kg/m² lower for people from South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds. A BMI of 30 to 34.9 may be considered if referral criteria are met through the specialist service itself. NICE also specifies a maximum of two years of treatment on this NHS route, and recommends reviewing whether to continue if less than 5% weight loss has occurred after six months on the maintenance dose.

Why is free NHS access so hard to get in practice?

Even people who meet the BMI and comorbidity thresholds often find that local services are oversubscribed. NHS England runs weight management injection programmes that include Wegovy, but commissioning varies by Integrated Care Board. A person in one area might be seen within three months; in another, the same person joins a list with no clear timeline. The two-year ceiling under TA875 also means the NHS pathway is designed as a time-limited intervention, with lifestyle support built in rather than ongoing prescription alone.

Demand has grown sharply since clinical trials showed semaglutide produced around 15% average weight loss over 68 weeks, results that, understandably, prompted a wave of interest. Is there any other way to get semaglutide at reduced or no cost?

There is no legal shortcut. Semaglutide sold without a prescription, through social media, or at prices far below the market rate is a serious safety risk. The MHRA has warned repeatedly about counterfeit weight-loss pens, some of which have contained insulin rather than the labelled medicine. If a seller is offering Wegovy cheaply with no clinical assessment involved, that is a red flag, not a bargain.
