The short answer is categorisation. Private health insurers in the UK have historically drawn a line between "medical" treatments and "lifestyle" ones, and weight management medicines have landed firmly in the second group for most providers. Wegovy is licensed for obesity (a recognised clinical condition) but insurers' policy documents have not always caught up with that framing. There is a publicly funded route to Wegovy. NICE recommended semaglutide (Wegovy) for NHS use in 2023 under TA875, but with strict conditions: it must be prescribed within a specialist weight management service, as part of a multidisciplinary programme, for a maximum of two years, and for adults with a BMI of 35 or above and at least one weight-related health condition. Lower BMI thresholds can apply for some ethnic backgrounds under UK guidance.
In practice, those specialist services have long waiting lists in most areas. Demand outstrips current capacity significantly. Meeting the NICE criteria does not translate to quick access; some patients wait a year or more for a first appointment. If you are referred by your GP, your local integrated care board determines which services are commissioned and when. Scotland, Wales and Northern Ireland operate under different frameworks with their own criteria.
