Wegovy Left in the Pen After Injecting: What the Remainder Means

The most common worry: did I inject the full dose?

The biggest misconception about Wegovy pens is that visible leftover liquid means the dose was incomplete. It does not. Novo Nordisk's pre-filled Wegovy pens are engineered with what pharmacists call an overfill: additional solution built into the cartridge to ensure the delivery mechanism can consistently push out the exact prescribed volume, accounting for the dead space in the needle and the mechanical tolerances of the plunger. When you hold the pen against your skin, press the button, and hold for the recommended count before withdrawing, you receive your full dose. The liquid you see afterwards exists because the pen was designed that way. It is not something that went wrong.

A useful way to think about it: the overfill is a quality-control buffer, not a second dose. This is standard practice across most pre-filled injection devices used in the UK, including insulin pens and other weekly injections. What you should watch for is whether the dose counter (on pens with a display) returned to zero, or whether you heard and felt the full click and held the pen in place for the count shown in the leaflet. What to do with the pen once you have finished your injection

Once your weekly injection is complete, the pen should be disposed of promptly and correctly. This is not optional: used sharps are a clinical-waste issue, and blunt or contaminated needles left in pens pose a real risk if they end up loose in household rubbish. In practice, the right route is a suitable sharps container, local councils in England collect these; your pharmacy may also accept them.

Remove the needle before disposal (following the steps in your Patient Information Leaflet) and cap it separately before placing it in the sharps bin. The pen body can then follow. Resist the temptation to save the device on the off-chance you might use the remaining solution: the pen is calibrated for one delivery only, any subsequent attempt would not deliver a reliable dose, and the integrity of the solution cannot be guaranteed once the needle has been attached and the device activated. One practical note: many people find it easiest to handle sharps disposal on injection day before the school run ends and the kitchen gets busy again. Keeping a small sharps bin in the same place as the fridge helps build the habit.

Storage of unused pens and what changes once a pen is in use

Wegovy pens that have not yet been activated must be stored in the refrigerator between 2°C and 8°C. Once you begin using a pen for a given week's dose, the storage conditions for any pen you have opened but not yet used are governed by the Patient Information Leaflet and the information on keeping Wegovy refrigerated correctly. The rules for an unused pen that has been left out briefly, for instance during travel or a busy morning, differ from the rules for a pen that has already been injected, so it is worth being clear which situation applies to you.

When to contact your prescriber rather than moving on

Most of the time, leftover liquid in a completed Wegovy pen is nothing to act on. There are, though, a handful of situations where the right response is to check in with a clinician before the next dose. These include: the pen did not click or the plunger did not depress fully; you injected but felt no resistance and are unsure solution was delivered; the pen was accidentally dropped before use and the cartridge looks cracked or cloudy; or you are experiencing unusual symptoms in the days following injection that make you wonder whether something went differently to usual.

Wegovy's active ingredient, semaglutide, is a prescription medicine, its dose is set by a prescriber for clinical reasons, not as a rough guide.
