Published injection-technique recommendations for subcutaneous medicines, including GLP-1 receptor agonists such as semaglutide, have shifted considerably over the past decade. Older practice favoured longer needles (8mm or more) on the assumption that greater depth guaranteed reliable subcutaneous delivery. Evidence has since reversed that view. Studies examining skin and subcutaneous tissue thickness across different body sites and BMI ranges found that, for most adults, a 4mm needle placed at 90 degrees reaches the subcutaneous layer reliably, without the risk of inadvertent intramuscular injection that comes with longer needles at the same angle.

The NHS medicines information for semaglutide points users to the Patient Information Leaflet for technique detail; the leaflet itself recommends injecting into the abdomen, thigh or upper arm, rotating sites with each dose. Needle depth is relevant to that process: too shallow and the medicine may pool under the skin surface; too deep and muscle delivery alters the absorption profile and increases bruising risk. A 4mm needle, used correctly at 90 degrees, avoids both problems for the majority of users.

Body composition matters here. Individuals with very little subcutaneous fat at a given site may benefit from lifting the skin slightly (a technique sometimes called a 'skin pinch') to ensure enough tissue depth. Does Wegovy's pen need a separate 4mm needle?

Injection comfort and why site rotation matters as much as needle size

Clinical trial data from the STEP programme, which evaluated semaglutide 2.4mg in thousands of adults, reported injection-site reactions as one of the noted adverse effects, typically mild redness, bruising or tenderness at the injection site. These are common with any subcutaneous medicine and are rarely serious, but they are more frequent when sites are not rotated properly.

The three approved sites for Wegovy are the abdomen (avoiding a 5cm zone around the navel), the front of the thigh and the outer upper arm. Rotating between them (and moving slightly within each site each week) allows tissue to recover between doses. At every stage, the physical act of injecting is the same: a clean site, the correct angle, a slow steady press of the injector, holding for the full count to ensure complete dose delivery.

A few details people often overlook: the pen should be at room temperature before you inject (take it from the fridge around 30 minutes beforehand), check the liquid is clear and colourless, and confirm the dose window shows the correct setting before you press.
