Wegovy on the NHS in England: eligibility, waiting lists, and your options

Step 1 — the NICE recommendation and what it actually requires

NICE published its guidance on semaglutide for weight management (TA875) in March 2023. The recommendation is positive, but it comes with conditions that narrow real-world access considerably. To qualify for NHS Wegovy in England, adults generally need a BMI of 35 or above plus at least one weight-related health problem — conditions such as type 2 diabetes, high blood pressure, dyslipidaemia, obstructive sleep apnoea or cardiovascular disease. If your background is South Asian, Chinese, Middle Eastern, Black African or African-Caribbean, UK guidance applies a threshold 2.5 kg/m² lower, so 32.5 and above. Critically, treatment must sit inside a specialist weight management service with a multidisciplinary team, dietitian, clinical psychologist, physician. It is not something a GP prescribes independently off the back of a consultation. And the cap is firm: NICE says stop after two years, or sooner if less than five percent of body weight has been lost after six months on the maintenance dose.

That last point matters more than most people realise. A medicine that works well for you can still be withdrawn at the two-year mark under NHS rules, leaving a gap that many patients then look to fill privately.

Step 2, getting referred and what the waiting list looks like

The route begins with your GP. They assess whether you meet the BMI and comorbidity thresholds and refer you to a tier 3 or tier 4 weight management service, the specialist multidisciplinary teams NICE requires. Your GP does not prescribe Wegovy themselves under TA875; they are the gateway, not the prescriber.

The problem is the waiting lists. Specialist services across England were stretched before semaglutide arrived; demand has risen sharply since. Some areas have waits measured in months, others in years. Referral criteria also vary slightly by integrated care board, so what gets you onto a waiting list in one part of England may not in another. Timing is a real factor here. If you are hoping to start treatment before a holiday, a planned surgery, or simply before the waiting list adds another year to your life on the wrong trajectory, the NHS route may not move fast enough. That is a practical truth, not a criticism.

Step 3, understanding what you receive when treatment is approved

Once accepted into a specialist service and started on Wegovy, you will be managed within the wraparound programme: regular contact with the team, dietary guidance, and structured activity support. The semaglutide itself follows the standard titration schedule, beginning at 0.25 mg weekly and increasing approximately every four weeks, under the supervision of your service, to the standard 2.4 mg maintenance dose. The higher 7.2 mg dose approved by the MHRA in 2026 is not currently part of the NHS pathway; it is only available privately.

Step 4, if the NHS route is not an option right now

Many people do not meet the current NHS criteria, or do meet them but cannot wait. A BMI between 27 and 34.9 with a weight-related condition is enough to qualify for a private prescription under Wegovy's licensed indications, even though it falls short of the NHS threshold.
