Wegovy on the NHS in Wales: the reality behind the headlines

The misconception: Wales follows the NHS England rollout

Almost every national headline about Wegovy on the NHS has described an England-specific programme. The phased rollout that began in June 2025, which is gradually opening up to people with higher BMIs and multiple weight-related conditions, sits under NHS England's commissioning authority. It does not bind NHS Wales, NHS Scotland or NHS Northern Ireland, each of which makes its own decisions about which medicines to fund, through which services and at what pace.

In practice, this means a patient in Cardiff and a patient in Bristol can be in very similar clinical situations and face very different NHS options. That is not a flaw specific to weight-management medicines; it reflects how devolved health services work. But it does mean that anyone in Wales who has read about GPs in England beginning to prescribe tirzepatide under the new GP contract should not assume the same is happening at their surgery in Swansea or Wrexham.

Wales does have specialist weight-management services, and Wegovy can in principle be prescribed within them where local health boards have chosen to fund it. The key phrase is 'where local health boards have chosen to fund it.' Provision is inconsistent and, in many areas, waiting lists for specialist services are long. What NICE TA875 says, and what it doesn't guarantee in Wales

NICE recommended semaglutide for weight management (TA875, published March 2023) for adults with a BMI of 35 or above and at least one weight-related comorbidity, or a BMI between 30 and 34.9 where a specialist referral would otherwise be considered. Lower thresholds apply for some ethnic backgrounds under UK guidance. Critically, TA875 also specifies that treatment should happen within a specialist weight-management service offering multidisciplinary support, and that it should not continue beyond two years. If fewer than 5% of body weight is lost after six months at the maintenance dose, the recommendation is to reconsider continuing.

NICE guidance is formally binding on NHS England. In Wales, the All Wales Medicines Strategy Group (AWMSG) advises on medicines funding, and it operates on its own appraisal cycle. Whether a NICE-recommended medicine becomes routinely funded by NHS Wales depends on AWMSG review and local health board decisions. This is why two people reading the same NICE summary can reach completely different conclusions depending on where they live.

Your GP in Wales can discuss your options, and some health boards do provide access to Wegovy through tier 3 or tier 4 weight-management services. The most honest thing to do is ask your GP directly what your local health board currently commissions, and whether a referral pathway exists. The licensed criteria for Wegovy are set by the MHRA, not by the NHS. Semaglutide is licensed for adults with a BMI of 30 or above, or 27 and above with a qualifying weight-related condition such as hypertension, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease or type 2 diabetes. Meeting the licensed criteria does not automatically mean NHS funding; it means a prescriber can legally consider the medicine for you.
