Pain Under Your Left Rib on Wegovy: Sorting Fact from Worry

The misconception: rib pain means Wegovy has damaged something

The fear most people arrive with is that the ache under their left rib is a sign of organ damage, kidney trouble, a spleen problem, or something the medicine has done that cannot be undone. In the vast majority of cases, that fear is unfounded. Semaglutide, the active ingredient in Wegovy, acts on GLP-1 receptors that are found throughout the gut. One of its principal effects is slowing gastric emptying: food lingers in the stomach longer than it normally would. That backed-up pressure can radiate upward and outward, settling in a spot that feels much more alarming than a stomach complaint usually does.

Bloating and gas amplify this further. When digestion slows, air that would ordinarily move on accumulates instead. The splenic flexure of the colon sits directly under the left rib, and when gas collects there (a well-documented phenomenon sometimes called splenic flexure syndrome) the result is a sharp or cramping pain that people frequently mistake for something more sinister. This does not mean every instance of left-sided rib discomfort can be waved away. It means the benign explanations are far more statistically likely, and understanding them helps you recognise what falls outside that normal range.

What does cause left-sided discomfort after a Wegovy injection

Setting aside the gas and pressure explanation, there are a handful of other mechanisms worth knowing about. Nausea often accompanies early doses of Wegovy, and the physical act of retching or repeated vomiting can strain the intercostal muscles, the small muscles between your ribs. That strain produces a localised, tender ache that worsens when you breathe deeply or move in certain directions. It is muscular, not visceral, and it resolves on its own.

Hiccupping and reflux, both reported side effects documented on the NHS's semaglutide page, can also produce diaphragm irritation that is felt as a catch or pressure below the left ribs rather than in the chest or throat where you might expect it. When left-sided pain is a reason to seek help promptly

The MHRA issued a Drug Safety Update in January 2026 specifically flagging acute pancreatitis as a known but uncommon serious effect of GLP-1 receptor agonist medicines. The pancreas has a tail that extends toward the left side of the abdomen, and pancreatitis characteristically produces severe, persistent pain that often radiates toward the back. It is usually accompanied by vomiting and does not settle with a change of position or over a few hours. There are other causes of left-sided rib pain that are entirely unrelated to Wegovy but that coincide with treatment, a chest infection, a musculoskeletal injury, or in rare cases a splenic issue. If the pain is sudden, severe, follows an injury, or is accompanied by breathlessness, fever, or shoulder-tip pain, treat it as an emergency. A sustained sharp pain on the left side that extends into the left shoulder (particularly after recent abdominal injury) can signal a splenic problem and warrants urgent assessment.

The short version: occasional mild discomfort under the left rib, coming and going with meals, that settles within a day or two, is almost always gastrointestinal.
