The published trial programme for semaglutide 2.4mg (including the STEP 1 study in the New England Journal of Medicine, which followed 1,961 adults over 68 weeks) recorded side effects rigorously. Cold intolerance does not appear as a frequently reported adverse event in those results. The Wegovy SmPC lists the predominant side effects as gastrointestinal: nausea, diarrhoea, vomiting, constipation and indigestion are common, especially around dose increases, and you can read more about feeling sick on Wegovy if nausea is something you're also navigating. Feeling cold is not in that primary list.
That doesn't mean the experience isn't real for the people who have it. It means the mechanism is indirect. Body fat is one of the body's most effective insulators. As fat mass decreases (sometimes substantially on a medicine that produced roughly 15% average weight reduction in STEP 1) the thermal buffer that kept you warm at your previous weight is simply smaller. One practical note: the timing of this sensation tends to follow the rate of weight loss rather than the dose level, which means it can emerge a few months in rather than at the very start, sometimes around the point people are consolidating on a maintenance dose.
A few other mechanisms are worth knowing about. First, fatigue. This tends to settle as the body adjusts, usually within the first few weeks of a new dose.

This is a common experience with any effective weight-loss approach and is not unique to GLP-1 medicines. Keeping protein intake adequate (something to discuss with your prescriber or a dietitian) supports lean muscle, which is metabolically active and contributes to warmth.
Third, and less commonly, some people develop iron-deficiency anaemia during periods of significant weight loss, particularly if their diet changes substantially. Anaemia is a well-known cause of feeling cold. If you're also feeling breathless, unusually pale, or your heart seems to race on minimal exertion, mention it at your next clinical review. Similarly, thyroid function changes can cause cold intolerance; if you have a thyroid history, your GP can check levels with a simple blood test.
Separating these possibilities matters because the responses are different: if it's fat loss, layering up more is the main answer; if it's anaemia or thyroid, investigation and treatment change the picture. Most cold sensitivity linked to weight loss is benign and self-limiting. A few situations warrant more urgent attention. Get medical help promptly if you experience severe, persistent abdominal pain that extends towards your back (with or without vomiting) as this could indicate acute pancreatitis, which the MHRA flagged in a January 2026 Drug Safety Update as a known, infrequent but potentially serious GLP-1 side effect. That's unrelated to cold specifically, but worth knowing as you read about any new symptom.

The body adjusts to a lower fat mass over time, and many people find the sensation of feeling cold gradually settles, or they adapt to it without it mattering much. Practical steps that help: prioritise protein and iron-rich foods in your reduced-calorie eating pattern; keep up strength-based activity, which supports muscle mass and metabolic rate; and layer clothing appropriately, especially in winter.
