Fever and temperature changes as Wegovy side effects: what's real, what's not

The misconception: that feeling hot or flushed means Wegovy is causing a fever

A lot of people starting Wegovy describe a sensation of warmth, mild flushing, or feeling 'off' in the first week or two. It is easy to mistake this for a temperature, especially if you are also dealing with nausea or fatigue at the same time. In reality, semaglutide's most common side effects are gastrointestinal (nausea, loose stools, constipation, indigestion) and the NHS medicines page for semaglutide does not list fever as a common or uncommon effect.

Feeling flushed can happen for a number of unrelated reasons: a viral illness that coincides with starting treatment, dehydration from early nausea reducing your fluid intake, or simply the physiological shift that comes with eating significantly less. None of these are fever caused by the medicine itself. A standard thermometer reading below 38°C is not a clinical fever, and an elevated temperature that appears alongside a cold or stomach bug most likely has nothing to do with Wegovy at all. That distinction matters, because the management is different.

When a temperature alongside Wegovy symptoms needs urgent attention

There are situations where a raised temperature in someone taking Wegovy should not be waited out. Acute pancreatitis is a known but infrequent risk with GLP-1 medicines; the MHRA issued a Drug Safety Update in January 2026 specifically highlighting this, noting that severe, persistent stomach pain that radiates to the back (with or without vomiting) should prompt urgent medical assessment. Pancreatitis can cause fever as part of its presentation, so if that combination of symptoms appears, call 111 or go to A&E rather than waiting for your next prescription review.

Gallbladder problems, including gallstones and cholecystitis, are also listed in the Wegovy prescribing information. Cholecystitis classically presents with right-sided or upper abdominal pain, nausea, and fever. Again, a temperature in this context is a warning sign, not a side effect to manage at home. Severe dehydration from persistent vomiting or diarrhoea can raise body temperature too, and dehydration on a GLP-1 medicine can put the kidneys under pressure faster than people expect. If you cannot keep fluids down for more than a few hours, that is a same-day concern.

Navigating side effects during the first weeks of treatment

Most Wegovy side effects peak in the days after a dose increase and settle within a week or two. Eating smaller portions, staying hydrated, and not lying down immediately after eating all help. Some people find eating a light meal before the weekly injection reduces next-day nausea; others prefer to inject on a Friday evening so any rough days fall over the weekend rather than a Monday morning.

What matters clinically is the difference between uncomfortable and concerning. Mild nausea: expected. Vomiting that prevents any fluid intake, pain that is severe and does not settle, or any symptom that feels genuinely alarming: act on it.
