What NICE guidance says about NHS access, and why most people don't qualify yet

The evidence base for these medicines is substantial. In the SURMOUNT-1 trial, tirzepatide produced an average body-weight reduction of around 20–21% over 72 weeks. Semaglutide 2.4mg (Wegovy) delivered around 15% in the STEP 1 trial. Both figures come from large, well-controlled studies published in the New England Journal of Medicine. That evidence is exactly why NICE recommended both medicines, but the NHS rollout is phased and the thresholds are high.

From June 2025, tirzepatide became available on the NHS for adults with a BMI of 40 or above who also have four or more specific weight-related conditions (high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, or type 2 diabetes). A second cohort (BMI 35 to 39.9 with four or more conditions) became eligible from around June 2026. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance. The trajectory is clear, but millions of people with BMIs in the 27–34 range, or fewer qualifying conditions, sit outside these NHS thresholds entirely. How private weight loss injection prices are structured in the UK

Private treatment is priced per pen, which equates to four weeks of treatment. Eli Lilly raised Mounjaro's UK list price from 1 September 2025 (the highest dose's list price moved from around £122 to around £330 per pen) and typical private prices since then have ranged roughly £149 to £375 per month by dose and provider, as widely reported in August 2025. Starting doses cost less than maintenance doses, which matters because treatment usually begins at the lowest strength and is titrated over several months.

That range tells you the floor and ceiling. What it doesn't tell you is what's included. Some providers quote a per-pen price that covers the medicine and nothing else. Consultation fees, prescription fees, and delivery can all be added separately, sometimes bringing the real monthly cost noticeably above the headline figure. One provider charges a consultation on top; another folds it in but charges per dose-review request. It adds up. Why 'cheapest' is the wrong question to ask about a prescription medicine

These are Prescription-Only Medicines. UK law requires a prescription following a clinical assessment, and the MHRA has warned publicly about fake weight loss pens being sold online, some containing insulin rather than the medicine on the label.
