Semaglutide tablets: what they are and what the evidence says

How the MHRA approval came about, and what it covers

When the MHRA licensed Wegovy tablets in June 2026, it drew on data from the OASIS 4 phase 3 trial: 307 adults with obesity or overweight plus at least one weight-related condition, without type 2 diabetes, followed for 64 weeks alongside lifestyle changes. Average weight loss on semaglutide reached around 13.6% compared with roughly 2.4% on placebo. Among participants who stayed fully adherent to their prescribed dose throughout, that figure rose to approximately 16.6%, though that higher number applies specifically to the adherent subgroup, not the trial population as a whole.

The licence covers adults with a BMI of 30 or above, or a BMI of 27 to 29.9 alongside at least one weight-related condition such as high blood pressure, raised cholesterol, or prediabetes. That is the same broad eligibility frame as the injectable Wegovy, but the tablet carries its own approval based on its own clinical programme. The approval was granted by the MHRA, which is the UK medicines regulator, not the European Medicines Agency. That distinction matters: the MHRA assessed the evidence independently after the UK left the EU regulatory framework, and its decision applies to England, Scotland, Wales and Northern Ireland. What semaglutide tablets actually are, the science without the jargon

Semaglutide is a GLP-1 receptor agonist. GLP-1 is a hormone your gut releases after eating; it signals fullness to the brain, slows the rate at which the stomach empties, and influences how your body manages blood sugar. Semaglutide mimics that signal in a sustained way, keeping the appetite-reduction effect going throughout the week with the injection, or throughout the day with the tablet.

The tablet version adds something the injection does not need: SNAC (sodium N-[8-(2-hydroxybenzoyl)amino]caprylate), an absorption enhancer that allows the semaglutide molecule (which would otherwise be broken down in the gut) to cross the stomach lining into the bloodstream. Without SNAC, oral delivery would not work. This is why the tablet has specific instructions: taken first thing in the morning on an empty stomach, swallowed whole with a small amount of plain water (up to around 120 ml), and nothing else (no food, no coffee, no other medicines) for at least 30 minutes afterwards. A useful cue for many people is to take it before the kettle has even finished boiling, then wait before making the first drink of the day.

The dose schedule and what 'titration' means in practice

Semaglutide tablets follow a step-up schedule designed to let the body adjust before reaching the therapeutic maintenance dose. The sequence runs: 1.5 mg, then 4 mg, then 9 mg, then the 25 mg maintenance dose. Each step requires at least one month before moving up; the prescriber decides the timing based on how you are tolerating the medicine, not a fixed calendar.

The most common side effects in the OASIS 4 trial were gastrointestinal: nausea, diarrhoea, constipation and vomiting were reported by a majority of participants on the active treatment, compared with fewer than half on placebo. These effects were generally mild to moderate and tended to ease as the body adjusted, particularly after the first few weeks at a new dose. NHS availability and what private prescribing currently looks like

As of summer 2026, Wegovy tablets are not available on the NHS. A NICE technology appraisal would be needed before NHS commissioning could begin, and that process had not started at the time of the MHRA approval. The injectable Wegovy has its own NICE recommendation (TA875), but that does not automatically extend to the tablet formulation.
