What BMI Do You Have to Be for Mounjaro?

The BMI numbers: what the licence says and what it doesn't

The UK marketing authorisation for Mounjaro sets the floor at a BMI of 30. That covers most people who would describe themselves as significantly overweight. The 27–29.9 band unlocks a second route: one or more weight-related comorbidities. Conditions that typically qualify include type 2 diabetes, prediabetes, high blood pressure, dyslipidaemia (raised cholesterol or triglycerides), obstructive sleep apnoea, and osteoarthritis, among others. You need only one of them alongside a BMI of at least 27.

Ethnic background adjustments exist because BMI is an imprecise tool across different populations. UK clinical guidance recognises that the health risks associated with excess weight emerge at lower BMI readings for people of South Asian, Chinese, Middle Eastern, Black African or African-Caribbean heritage, so the thresholds shift 2.5 kg/m² downward throughout. A person in that group with a BMI of 27.5 and a qualifying condition may therefore be eligible where the standard cutoff would say otherwise.

Why BMI is just the starting point, not the finish line

Reaching the BMI threshold gets you to the door of a prescriber, it doesn't guarantee you'll walk through it. A responsible prescriber reviews your whole health profile: current medications, medical history, family history of certain conditions, and any contraindications listed in the Mounjaro prescribing guidance.

Certain situations mean Mounjaro is not appropriate regardless of BMI. Active pancreatitis or a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 are examples where the medicine is contraindicated. NHS thresholds versus private treatment: they are not the same

This distinction confuses a lot of people, so it's worth stating plainly.
