How overweight do you have to be to get Mounjaro?

The myth worth correcting first: you don't have to be very obese to qualify

A lot of people assume Mounjaro is reserved for those with a very high BMI, the sort of weight that makes everyday movement difficult. That assumption puts people off even asking. In reality, the licensed threshold in the UK starts at a BMI of 27, which sits in the overweight range rather than the obese range, provided you have a weight-related health condition alongside it.

The Mounjaro SmPC, reviewed by the MHRA, sets out two qualifying routes. Route one: BMI 30 or above, with no other condition required. Route two: BMI 27–29.9, with at least one of the conditions that weight is known to worsen, type 2 diabetes, prediabetes, high blood pressure, high cholesterol, obstructive sleep apnoea, or osteoarthritis, among others. The other thing worth knowing: BMI is calculated from height and weight, and it tends to underestimate health risk in some ethnic groups. UK guidance acknowledges this. For people of South Asian, Chinese, Middle Eastern, Black African or African-Caribbean backgrounds, the qualifying thresholds are 2.5 kg/m² lower across the board, so 24.5 instead of 27, and 27.5 instead of 30. What a prescriber actually looks at, beyond the number on the scale

Meeting the BMI threshold opens the door; it does not walk you through it. Every consultation at nume is reviewed personally by a GPhC-registered Independent Prescriber, a real clinician reading your answers, not software running a tick-box check. They look at your BMI alongside your medical history, any medicines you already take, and the conditions that may either support or rule out Mounjaro.

Certain things require careful discussion before any prescription is issued. A history of pancreatitis, medullary thyroid carcinoma or Multiple Endocrine Neoplasia type 2 means Mounjaro is not appropriate. Pregnancy, breastfeeding, or actively trying to conceive also rules it out. Under-18s are not licensed to use it. These aren't bureaucratic hurdles, they reflect genuine clinical reasoning about who benefits and who could be harmed.

NHS eligibility versus private eligibility, they are not the same thing

This is where things get genuinely confusing, because two separate sets of criteria exist side by side. The licensed eligibility described above (BMI 30, or 27 with a condition) is the standard for private prescriptions. NHS eligibility follows a much narrower phased rollout set by NICE's appraisal of tirzepatide (TA1026) which currently requires a BMI of 40 or above plus four of five specific conditions (high blood pressure, dyslipidaemia, sleep apnoea, cardiovascular disease, type 2 diabetes). A second cohort covering BMI 35–39.9 with four conditions opened in June 2026.

The practical upshot: many people who qualify for a private prescription through a regulated pharmacy do not yet qualify for NHS Mounjaro, and may be waiting years for those criteria to reach them. That gap is why services like ours exist. What happens if you're not sure whether you qualify

The honest answer is that the only way to find out is to go through a proper clinical assessment. You might be surprised. People who have spent years believing their BMI was "not high enough" sometimes discover they meet the BMI-plus-condition route without realising it, because a condition on the qualifying list was diagnosed years ago and quietly forgotten about.
