Start journey Learn moreWhen you start Wegovy, a reduced-calorie diet built around protein, fibre and steady hydration gives the medicine the best environment to work. Clinical trial participants who combined semaglutide with structured lifestyle changes lost significantly more weight than those on placebo — around 15% of body weight over 68 weeks on average, according to the STEP 1 trial published in the New England Journal of Medicine. What you eat in those first weeks shapes how well you tolerate the treatment and how quickly you feel the benefits. Wegovy is a prescription-only medicine; a prescriber reviews whether it is clinically appropriate for you before it can be issued.

Wegovy works by activating GLP-1 receptors in the gut and brain, slowing the rate at which food leaves the stomach and reducing the hunger signals that usually drive you back to the kitchen an hour after eating. That slower transit time is largely why the medicine is so effective. It is also why certain foods that you'd normally tolerate perfectly well can suddenly feel like too much. High-fat meals, very large portions and rich sauces sit in a slowed gut for longer, and nausea often follows. The NHS medicines page for semaglutide lists nausea, vomiting and diarrhoea as the most common side effects, particularly in the early weeks.
This is not a sign the treatment is failing. It is the medicine doing exactly what it was designed to do. Adjusting what and how much you eat is the practical response, not a workaround.
Practically: aim for three smaller meals rather than two large ones. Take your time eating. Putting your fork down between bites sounds like old advice, but it has real relevance here because the brain's satiety signal takes around 20 minutes to register and Wegovy amplifies that lag. Stopping before you feel full is often the right call.
If you are wondering what to eat when you are on Wegovy, the short answer in these early weeks is to lean on plain chicken or fish, eggs, Greek yoghurt, cooked vegetables, rice, oats and lentils, while spacing out or reducing anything deep-fried, creamy sauces, processed snacks and alcohol, which can worsen nausea and adds calories with no nutritional benefit.
One misconception worth addressing gently: some people starting Wegovy assume that because they are eating so much less, they should just eat whatever they fancy in smaller amounts. That logic is understandable, but it misses something important. When a body loses weight quickly, it draws on muscle as well as fat unless protein intake is kept deliberately high. Trial data consistently show that preserving lean mass requires adequate protein even as overall calories fall.
A practical target for most adults is around 1.2 to 1.6 grams of protein per kilogram of body weight per day, though your prescriber or a registered dietitian can give you a figure specific to your situation. Spreading it across meals works better than front-loading it, because the body can only use a certain amount at once.
Good sources that tend to sit well on Wegovy: eggs scrambled softly, cottage cheese, tinned fish, plain poultry, tofu and pulses. Protein shakes can fill a gap when appetite is very low, but whole food sources are preferable where tolerated. If you are finding it genuinely difficult to eat enough, that is worth raising with your clinical team rather than just pushing through.
Our broader Wegovy food guide covers long-term dietary patterns once you are past the initial adjustment phase.
Nausea, bloating and reflux are most common in the first four to eight weeks and usually settle as the body adjusts. Certain food choices reliably make them worse. NHS England's guidance on weight-management injections notes that GI symptoms are the main reason people consider stopping early, and dietary adjustments often resolve them without any change to the dose.
The main culprits: very fatty or fried foods slow digestion even further on top of what the medicine is already doing. Spicy food irritates a stomach that is already working differently. Fizzy drinks add gas to a gut that may already feel full. Large volumes of food at once (even healthy food) can trigger nausea simply by volume.
What tends to help: eating upright and staying upright for at least 30 minutes after a meal, eating slowly, choosing warm rather than very hot or very cold foods in the early weeks, and keeping portions modest. Plain crackers or dry toast can help settle nausea if it strikes between meals.
On the question of what to eat while taking Wegovy more generally, the same principles apply across the treatment period, but the early weeks are when food choices have the most visible impact on how you feel day to day.
For context on how Wegovy fits into a wider weight-loss plan, the treatment overview page sets out the full picture. If you are still exploring whether this is the right route, the Wegovy pillar page covers the clinical background in detail. Questions about cost are addressed separately on the Wegovy pricing and access page.
By weeks four to six, most people find a rhythm. Nausea has usually eased, appetite is noticeably lower than before treatment, and meals naturally become smaller. The risk at this point is not overeating, it is under-eating, particularly of the nutrients that matter most. A sustainable pattern for most people on Wegovy looks something like this: a protein-led breakfast (eggs, yoghurt, smoked salmon), a lunch built around vegetables and a lean protein source, and a lighter evening meal. Snacks are rarely needed and often not wanted, but if hunger does appear mid-afternoon, something small and protein-rich (a handful of nuts, a boiled egg) works better than reaching for something processed.
Alcohol deserves a mention: it interacts poorly with a slowed digestive system, tends to lower food inhibitions and adds empty calories. Reducing it during treatment is genuinely worth doing.
The dietary guidance for ongoing Wegovy use goes further on long-term patterns, including how to maintain good nutrition as doses increase. And if you have specific questions about your individual situation, our clinical support team is available seven days a week.
Wegovy is a prescription-only medicine. If you are thinking about starting treatment, check your eligibility with our prescribers, consultations are free, reviewed the same day by a GPhC-registered clinician, and there is no obligation to proceed.
