NICE's appraisal of tirzepatide (TA1026, December 2024) noted that in clinical trials the 15mg dose produced the largest average weight reductions, around 20–21% of body weight over 72 weeks, based on the SURMOUNT-1 trial published in the New England Journal of Medicine. One honest note: if you have reached 15mg, your prescriber has already taken you through several months of treatment and dose increases. Getting to this point represents a clinical journey, not just a transaction. The cost at this stage feels significant, and that's a fair reaction.
When a provider advertises a per-pen price, that number often covers the medicine alone. What it may not include: the initial consultation fee (some services charge £30–£60 separately), the prescription charge, delivery costs, and any fee for follow-up support or dose-review aftercare. A quoted price of, say, £299 for the pen can become materially higher once those elements are added. That's not a criticism of any particular approach, it's just an accurate description of how private medicine pricing frequently works.
There's also the question of clinical oversight. At the 15mg stage, a responsible service should review evidence that the dose increase is appropriate, not simply process a repeat. The NICE tirzepatide guidance (TA1026) explicitly notes that treatment should be reviewed if weight loss is insufficient, which is a clinical judgement, not an automated decision. If a service is not doing that work, it has to be excluded from somewhere, and it is usually the price.

The MHRA has warned publicly about fake weight-loss pens circulating through unverified online sellers. In a 2025 enforcement operation, around 20 million doses of illegally traded weight-loss medicines (with a reported street value of approximately £45 million) were seized, and the first illicit UK site producing counterfeit tirzepatide pens was raided. Counterfeit pens have contained insulin in place of the labelled medicine. That isn't a theoretical risk.
For a prescription medicine, the cheapest listing answers the wrong question. The right questions are: is this pharmacy registered with the GPhC? Is a real prescriber reviewing my case? Is the supply chain licensed? Those checks don't cost you extra at a regulated service, they're built in.
