Semaglutide works partly by slowing gastric emptying — the rate at which food moves from your stomach into the small intestine. That slower transit is part of how Wegovy reduces appetite, but it also means the stomach holds its contents, and its acid, for longer than usual. When stomach pressure builds, or when you lie down before the stomach has properly cleared, acid can track back up the oesophagus. The result is that burning sensation behind your breastbone that most people recognise immediately as heartburn.
The NHS medicines page for semaglutide lists heartburn (indigestion) alongside nausea, burping, and reflux as known gastrointestinal effects. They sit in the same family of symptoms because they share the same root cause. What varies is how strongly any individual feels them. Factors that tend to make reflux worse on Wegovy include eating large portions, lying flat within two or three hours of a meal, and foods that already relax the lower oesophageal sphincter, caffeine, alcohol, fatty or fried food, mint, chocolate. None of those are banned, but they're worth factoring in.
The good news is that for most people these symptoms peak in the first week or two at a new dose and then ease considerably. The dose-escalation schedule exists precisely to give your digestive system time to adapt. Pushing through a dose increase faster than your prescriber recommends is one of the main reasons heartburn stays bad rather than settling.
Small, practical changes make a real difference. Eating smaller meals more frequently (rather than three large ones) keeps stomach pressure lower. Sitting upright for at least two to three hours after eating gives acid less opportunity to travel upwards. Propping the head of the bed up slightly (even a folded pillow under the mattress) helps overnight symptoms for people who find lying flat triggers them.
On the food side, it's worth keeping a brief mental note of what seems to make things worse for you specifically. Not everyone has the same triggers. For many people on Wegovy it's the combination of a meal that's larger than their appetite now actually wants, eaten quickly, followed by sitting on the sofa, all three together amplify the reflux that any one of them might not have caused alone.
Over-the-counter antacids or alginates (such as those that form a raft on top of stomach contents) are generally used by people managing reflux, but whether they're appropriate alongside your specific treatment is a question for your prescriber or pharmacist. If symptoms are persistent rather than occasional, your prescriber may discuss whether a proton pump inhibitor or other medication could help, that is a clinical conversation, not something to self-initiate long-term.

Most heartburn on Wegovy is uncomfortable but not dangerous. The symptom to act on promptly is that severe, persistent upper abdominal pain spreading to the back, it is distinct from ordinary heartburn and requires urgent medical help rather than an antacid.
Other symptoms that warrant calling your GP or 111 rather than waiting: difficulty swallowing, vomiting that won't stop, signs of significant dehydration (very dark urine, dizziness, confusion), or any symptom that genuinely alarms you. Ordinary reflux and heartburn (the burning sensation after meals, the burping, the slight discomfort when lying down) sit in a different category. Unpleasant, yes; reason to stop without talking to your prescriber first, generally not.
