Semaglutide works partly by slowing gastric emptying, the rate at which food moves from the stomach into the small intestine. That mechanism is useful for reducing appetite and stabilising blood sugar, but it also means food and acid sit in the stomach longer than usual. When the stomach stays fuller for longer, the pressure on the lower oesophageal sphincter (the valve that keeps acid from travelling upward) can increase, leading to reflux and the burning discomfort most people call heartburn.
It is worth knowing that this is not a sign the medicine is doing something wrong. It is the same physiological process that makes Wegovy effective, producing an unwanted effect in a subset of people. Most cases are mild to moderate and tend to ease as the body adjusts, particularly after a new dose level has had a few weeks to settle.

Beyond portions, a few habits are worth building in: avoid lying down for at least two to three hours after eating; prop the head of your bed a few inches if night-time symptoms are the issue; cut back on foods that relax the lower oesophageal sphincter (caffeine, alcohol, fatty or spicy foods, chocolate, mint); and opt for plain water between rather than during meals. Tight waistbands also add abdominal pressure, looser clothing around mealtimes is a small but real adjustment.
Over-the-counter antacids (such as those containing calcium carbonate or sodium alginate) can offer quick relief for occasional symptoms and are generally compatible with Wegovy. Yes, and understanding this timing can take the alarm out of a flare. Heartburn and reflux symptoms on Wegovy are most likely to spike in the days immediately following a dose increase, then gradually subside. The pattern follows the same arc as nausea: worst in the first week or so after stepping up, better by weeks two to four. Most heartburn on Wegovy is mild, predictable and manageable. Some is not. Get prompt medical help if you notice heartburn that is severe and does not ease with standard remedies, any difficulty or pain when swallowing, vomiting that is persistent or contains blood, or unexplained weight loss beyond what treatment would explain. These can indicate conditions unrelated to Wegovy, or, rarely, complications that need investigation.
