How to Attach a Needle to Your Wegovy Pen Correctly

Each Wegovy pen has a threaded tip: needles screw on and must be attached fresh for every injection — never reuse a needle.
The needle has two caps: an outer cap (for handling) and an inner cap (protects the needle tip before use); both are removed just before you inject, not before.
Needle length for Wegovy pens is typically 4–8 mm; the right length for you is worth confirming with your prescriber. Our guide on how long a Wegovy needle is covers this in detail.
Used needles go straight into a sharps container, never into household waste or recycling.

Attaching a needle to a Wegovy pen takes about thirty seconds, but doing it right matters for a comfortable, effective injection. The process is the same across all Wegovy dose strengths: peel the paper tab from a fresh needle, push the capped needle straight onto the pen's thread, and screw it on until it sits firm. The pen is a prescription-only medicine — your prescriber and the Patient Information Leaflet that comes with every box are your primary references for anything specific to your situation. What follows is a practical, step-by-step overview to help you feel confident before your first injection.

Attaching, Using and Removing the Needle Safely: The Decisions That Matter

The actual decision: which needle to use before you even attach it

Before anything goes near the pen, you need the right needle. Wegovy pens use standard pen needles with a universal thread, so in principle many compatible needle brands fit, but not all needle lengths suit all injection sites or all people. A common misconception is that shorter needles are always better. In practice, the right length depends on your injection site and tissue depth, and it is your prescriber who advises on that, so reading about how long a Wegovy needle is can help you go into that conversation more informed. If you have questions, the size of the Wegovy needle is worth reading before you start.

Once you have the correct needle, check the packaging is intact and the needle is within date. A compromised needle wrapper is a reason to open a fresh one. The NHS guidance on semaglutide, published on NHS.uk, reinforces that injection technique and equipment hygiene are part of safe use of the medicine.

Store spare needles at room temperature, away from moisture. The pen itself lives in the fridge between uses.

Screwing the needle on: the step-by-step

Take the pen out of the fridge around 30 minutes before your injection, cold medicine can sting more. Wash your hands thoroughly. Then:

1. Peel the paper backing from a new needle. Do not remove either cap yet. 2. Hold the pen upright and align the needle straight with the pen's tip, no angle. 3. Push the needle onto the thread and screw clockwise until it is snug. Firm, not forceful; over-tightening can make the outer cap harder to remove. 4. Pull off the outer cap and set it aside, you will need it later. 5. Pull off the inner cap and discard it. The needle is now ready. Do not recap with the inner cap.

If you are taking Wegovy for the first time and want a broader picture of how the injection process fits into your treatment, the full Wegovy overview explains what to expect from the first pen onwards.

After the injection: removing the needle correctly

Once you have injected and held the pen in place for the count recommended in the leaflet, it is time to remove the needle. Replace the outer cap over the needle (use the cap, not your fingers) and unscrew the needle from the pen. It should come away cleanly. Place it straight into a dedicated sharps container; this is a legal requirement, not just good practice.

Never leave a needle attached to the pen between injections. Leaving it on exposes the medicine to air and risks contamination or leakage that can affect the dose. Replace the pen cap and return it to the fridge.

Rotate your injection site each week. Abdomen, thigh and upper arm are all licensed sites for subcutaneous injection. Repeatedly using the same small patch of skin can cause fatty lumps (lipohypertrophy) that slow absorption. Your prescriber can show you a rotation pattern if you are unsure.

Needle tips and what to do if something feels wrong

Most people find the injection straightforward after the first couple of attempts. A few practical points our prescribers hear regularly: if the needle feels blocked, do not push harder, check the cap is fully removed and that the needle is screwed on straight. A bent needle should be discarded and replaced before injecting. If you notice redness, swelling or persistent pain at the injection site beyond a day or two, mention it at your next clinical review.

For guidance on the finer points of needle handling, our needle tips for the Wegovy pen page covers common questions in more depth, including how to handle needle anxiety. The Wegovy needles overview is useful if you are sourcing spares or want to understand compatibility. Information on Wegovy needle types can help clarify your options before speaking to your prescriber.

If you ever have a concern about your injection that cannot wait, contact your prescriber or call 111. Report any suspected side effects using the MHRA's Yellow Card scheme, it applies to Wegovy just as to any other medicine.

Wegovy is a prescription-only medicine. Private treatment starts with a clinical consultation, if you are still in the process of starting or exploring your treatment options, check your eligibility with our prescribers at any point.
