How to take Mounjaro: injection technique, timing and what to expect

Choosing where to inject, and why the decision matters more than most people expect

Mounjaro can be injected into three sites: the abdomen (at least a few centimetres away from your navel), the front of your thigh, or the back of your upper arm. All three are clinically acceptable, but the practical experience differs. The abdomen is the easiest site to reach and visualise yourself, which is why most people start there. The thigh works well when your hands need a flat surface. The upper arm is manageable but usually easier with a second person for the first few attempts.

The rule that makes a real difference is rotation. Rather than picking your preferred spot and returning to it every week, move systematically, different quadrants of the abdomen, alternating thighs, or switching between sites altogether. Repeated injections into the same small area can cause localised thickening of the skin (lipohypertrophy), which affects how the medicine is absorbed. You can check in under a minute: run a fingertip across the skin at your usual injection site before each use; if it feels firmer or lumpier than the surrounding area, move at least a centimetre away, or choose a different site that week.

The actual injection, a step-by-step decision tree

The KwikPen is designed to make each step deliberate. Before you begin, take the pen from the fridge, put the cap on the counter and let it reach room temperature for about 30 minutes, cold medicine stings more than it needs to. Wash your hands thoroughly. If you are using an alcohol swab to clean the skin, let it dry completely before injecting; alcohol residue can cause stinging and may interfere with the skin's natural barrier.

Remove the inner needle cap. You do not need to prime the pen or expel a drop of medicine first, Mounjaro KwikPens are pre-set. Pinch the skin gently if the tissue is thin, position the pen flat against the skin at a 90-degree angle, press and hold the button until you hear a click, then keep the pen held in place for a slow count of ten. This pause matters: releasing the pen too quickly can mean the full dose hasn't delivered. A small red indicator in the viewing window confirms the dose is complete. Do not rub the site afterwards.

Timing, missed doses and what happens if you need to change your day

Consistency on the day of the week matters, but the time of day does not. Pick whichever day fits your routine (many people anchor it to a regular activity) and stick to it. Mounjaro can be taken with or without food.

If you miss a dose and it has been five days or fewer since your scheduled injection, take it as soon as you remember, then return to your normal weekly schedule. If more than five days have passed, skip that dose and resume on your usual day the following week. Never inject two doses in the same week to make up for a missed one. Side effects are most common after starting or after a dose increase. Nausea, loose stools and constipation are the most frequently reported; they usually ease within days to two weeks as the body adjusts. Storage, travel and the question of cost

Unopened pens live in the fridge. Once you are using a pen (meaning the needle cap has been removed for the first time) the storage window changes, and the precise duration and temperature conditions are set out in your Patient Information Leaflet. The short version is this: follow the leaflet for your specific pen; when in doubt, keep it refrigerated and protected from light.

Travelling with Mounjaro is manageable with a little planning. An insulated cool bag with an ice pack (never direct contact with ice) works for short journeys; for longer trips or flights, a reusable pharmaceutical cooler is worth the investment.
