The Wegovy tablet is taken once daily, first thing in the morning, on an empty stomach with a small amount of plain water, at least 30 minutes before any food, drink or other oral medicines. It reaches a 25 mg maintenance dose through a gradual escalation starting at 1.5 mg. In the OASIS 4 phase 3 trial, participants lost an average of around 13.6% of their body weight over 64 weeks. Among those who stayed fully adherent to the schedule, the average was closer to 17%. The most common side effects were gastrointestinal and were generally mild to moderate.

Step 3: Know what Mounjaro (the tirzepatide injection) actually involves

In the SURMOUNT-1 trial, published in the New England Journal of Medicine, participants using the 15 mg dose lost an average of around 20 to 21% of their body weight over 72 weeks. The trial randomised 2,539 adults, and the findings underpinned NICE's recommendation of tirzepatide in December 2024. A subsequent head-to-head trial, SURMOUNT-5, found tirzepatide produced greater average weight reduction than semaglutide 2.4 mg over 72 weeks.

Both Mounjaro and the Wegovy tablet are licensed for adults. For Mounjaro, the SmPC sets out eligibility as a BMI of 30 or above, or 27 or above alongside at least one weight-related condition such as high blood pressure, high cholesterol, type 2 diabetes or obstructive sleep apnoea. For the Wegovy tablet, the MHRA licensed it for adults with a BMI of 30 or above, or 27 to 29.9 with at least one weight-related condition.
