Wegovy (semaglutide) is available in five strengths in the UK — 0.25mg, 0.5mg, 1.0mg, 1.7mg, and 2.4mg — taken once weekly as a subcutaneous injection, with each dose stepped up roughly every four weeks by your prescriber. Since April 2026 a 7.2mg pen has also been approved by the MHRA for adults with a BMI of 30 or above. Getting the right Wegovy strength: the decision you're actually making

The strengths exist to protect you, not to upsell you

The titration schedule for Wegovy isn't arbitrary. Starting at 0.25mg gives your digestive system time to adjust before the dose climbs. That starting strength isn't doing much therapeutically, its job is to reduce the nausea and gut discomfort that would otherwise lead people to stop altogether. The steps through 0.5mg, 1.0mg and 1.7mg are each held for around four weeks before moving on, and 2.4mg is the standard long-term maintenance dose confirmed in the STEP 1 trial, where participants lost an average of around 15% of body weight over 68 weeks, as published in the New England Journal of Medicine.

The higher 7.2mg dose (approved by the MHRA in April 2026 and available as a single dedicated pen) is for adults with a BMI of 30 or above and must be reached by titrating up through the established schedule. No prescriber will hand you 7.2mg on day one. Nor should they. The legal route to any Wegovy strength in the UK

There is one legal way to obtain Wegovy in the UK: with a valid prescription issued by a qualified prescriber following a clinical assessment. That applies to the 0.25mg starter pen and to the 7.2mg pen equally. The MHRA has warned publicly about fake weight-loss injection pens sold online and via social media, seized counterfeits have included pens containing insulin rather than semaglutide, with hospitalisations following. Enforcement in 2025 led to around 20 million doses of illegally traded weight-loss medicines being seized, worth an estimated £45 million. The risks are not theoretical.

When you're looking at any online pharmacy, you can verify it takes 30 seconds: search the GPhC pharmacy register, enter the pharmacy's name or registration number, and confirm it holds a valid premises registration. If it doesn't appear, stop. Red flags to watch for: any seller offering these medicines without a prescription, prices far below the market rate, or discount codes pushed through social media. Those are not bargains. Report anything suspicious through the MHRA's Yellow Card scheme, which accepts reports from patients as well as clinicians.

How eligibility for Wegovy is assessed

The licensed criteria for Wegovy cover adults with a BMI of 30 or above, or a BMI of 27 to 29.9 alongside at least one weight-related condition, things like hypertension, high cholesterol, obstructive sleep apnoea or type 2 diabetes. For certain ethnic backgrounds, including South Asian, Chinese, Middle Eastern, Black African and African-Caribbean backgrounds, UK guidance applies thresholds that are 2.5 kg/m² lower. Meeting those numbers is a starting point, not a guarantee of a prescription. A prescriber looks at the whole picture: your medical history, current medicines, any conditions that would make Wegovy unsuitable, and what you're hoping to achieve. Wegovy isn't licensed for people who are pregnant, breastfeeding or actively trying to conceive, and it isn't licensed for under-18s. Anyone with certain thyroid or pancreatic conditions will be assessed carefully.
